Project Hazard Identification, Risk Assessment and Control (HIRAC) Form <br />37 <br />BBS is an approach to safety management that focuses on employee behaviors which may increase the <br />risk of an accident. The approach assumes that employee behaviors often precede an incident and <br />therefore modifying behavior can prevent an incident. <br />SLR’s BBS program involves reporting the following HSE-related leading indicators which can assist with <br />identifying and correcting unsafe conditions and behaviors: <br />• First Aids; <br />• Near Misses; <br />• Hazard Identifications; and <br />• Safety Observations / Conversation (SO/SC) <br />The SLR SSO should conduct at least one SO/SC at each project site during the course of field activities. <br />Completed forms shall be forwarded to the Project Manager and HSE Manager for review. <br />Field personnel are encouraged to make safety observations and transmit any safety observations to the <br />SLR SSO using the SO/SC Form (see Appendix A). <br />Field personnel are also expected to document and report any first aid cases, near misses or identified <br />hazards to the SLR SSO utilizing SLR’s Incident / Near Miss / Hazard Identification Form (see Appendix A). <br />11.0 SLR HSEP Completion / Review / Signoff <br />All HSEPs must be reviewed and signed by the SLR project manager and the office HSEC or their <br />designate. <br /> <br />Sarah Bortz <br /> <br /> <br /> 3/9/2021 <br />Project Manager Signature Date <br /> <br />Chris Ingalls <br /> <br /> 3/9/2021 <br />HSE Coordinator <br /> <br /> Signature Date <br />NOTE: Some of our clients have very specific HSE contract and program requirements that we must <br />adhere to.
