Violation: PRC 44014(b) The tonnage of waste received/accepted at this facility exceeded the permitted <br />maximum tonnage of 46,080 tons per week, not exceeding 8668 tons per day. The weekly tonnage received at this <br />facility was as following: <br />The week of 6/20/05 <br />49,534.36 tons <br />The week of 6/27/05 <br />49,179.20 tons <br />The week of 7/04/05 <br />42,670.24 tons <br />The week of 7/11/05 <br />52,275.40 tons <br />The week of 7/18/05 <br />54,778.81 tons <br />Between 6/20/05 and 7/23/05, the week tonnage received at this facility exceeded the permitted maximum tonnage <br />every week except the week of 7/04/05. Also see item 2 for the daily tonnage that also exceeded the permitted <br />maximum tonnage. <br />a. The amount of waste received at this facility shall not exceed the permitted maximum tonnage of <br />46,080 tons per week, not exceeding 8,668 tons per day. <br />b. Also see inspection dated 4/20/05, 5/25/05 and 6/24/05 for previous violation of this section. <br />4. Violation: T27CCR21600 RDSI has not been amended to include information listed in <br />T27CCR21600(b)(6)(B), the ADC usage and waste to cover ratio. <br />a. Submit application to amend RDSI to include the above information to LEA. <br />b. Also see inspection dated 4/20/05, 5/25/05 and 6/24/05 for previous violation of this section. <br />5. AC T27CCR20590 The truck drivers were not wearing safety equipment, vest or/and hardhat, <br />while working outside the vehicle near active face area. <br />a. Ensure that all personnel onsite wear safety vest, hard hat and other safety equipment. <br />b.
