T <br />STATE OF CALIFORNIA California Integrated Waste <br />CIWMB 3 (Rev. 4/04) Management Board <br />INSPECTION REPORT ADDENDUM PAGE 3 OF 4 <br />AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br />SITE NUMBER: 39 -AA -0015 INSPECTOR: Michael Kith <br />SITE NAME: Forward Inc. <br />RECEIVED BY <br />INSPECTION DATE: 07/27/05 OPERATOR <br />COMMENTS: <br />a. Cover all exposed waste. Provide minimum of 6 inches of compacted soil cover. <br />b. Also see previous inspection reports for previous area of concern of this section. <br />7. Violation: T27CCR20690(b)(3) Food waste and residential waste found in the green waste destined for <br />ADC usage. This waste (the mixture of food waste, residential waste and green waste) meets <br />the definition of garbage, rubbish and residential waste as defined in T14CCR17225.30, <br />T14CCR17225.59 and T14CCR17225.57, respectively. <br />a. All green waste used as ADC shall meet the requirement set forth in T27CCR20690. <br />b. Also see inspection dated 4/20/05, 5/25/05 and 6/24/05 for previous violation of this section. <br />8. AC T27CCR20750 The north slope of old Austin Road Landfill where leachate leaked had not <br />repaired. <br />a. Repair the slopes. <br />b. According to the operator, that area will be repaired when the final cover is installed this summer. <br />c. Also see inspection dated 2/24/05, 3/30/05, 4/20/05, 5/25/05 and 6/24/05 for previous area of concern of this section. <br />9. AC T27CCR20820 Erosions on the slopes of old Austin Road Landfill. <br />a. Repair all erosions. <br />b. According to the operator, that area will be repaired when the final cover is installed this summer. <br />10. AC T27CCR20870 No random load checking was performed in the green waste area. <br />a. Provide random load checking in that area. <br />b. Ensure that all hazardous waste and unacceptable waste are removed. <br />1 l . Violation T27CCR20919.5 The concentrations of methane gas were as following: <br />In GP9: 51 % in shallow 45.2 % in middle 0.2% in deep <br />In GP12: 55.5 % in shallow 50.5 % in middle 38.2 % in deep <br />a. Ensure that the concentration of explosive gas at the property boundary is below 5 % by volume. <br />b. Follow directions listed in T27CCR20919.5(c) and in the Amended Notice and Order 03-01. <br />c.
