Continued: <br />Michael Kith <br />operation requires prior approval from LEA and concurrence from the Board. <br />c. Also see inspection report dated 4/20/05, 5/25/05, 6/24/05, 7/27/05 and 8/26/05 for previous violation of this <br />section. <br />3. Violation PRC 44014(b) The tonnage of waste received/accepted at this facility exceeded the permitted <br />maximum tonnage of 46,080 tons per week, not exceeding 8668 tons per day. The weekly tonnage received at this <br />facility was as folloNN ing: <br />The week of 9/05/05 49,034.98 tons <br />The week of 9/12/05 48,797.94 tons <br />The week of 9/19/05 49,394.10 tons <br />Between 9/05/05 and 9/24/05, the week tonnage received at this facility exceeded the permitted maximum tonnage <br />every week. Also see item 2 for the daily tonnage that also exceeded the daily permitted maximum tonnage. <br />a. The amount of waste received at this facility shall not exceed the permitted maximum tonnage of <br />46,080 tons per week, not exceeding 8,668 tons per day. <br />b. Also see inspection dated 4/20/05, 5/25/05, 6/24/05, 7/27/05 and 8/26/05 for previous violation of this section. <br />4. Violation T27CCR21600 RDSI has not been amended to include information listed in <br />T27CCR21600(b)(6)(B), the ADC usage and waste to cover ratio. <br />a. Submit application to amend RDSI to include the above information to LEA. <br />b. Also see inspection dated 4/20/05, 5/25/05, 6/24/05, 7/27/05 and 8/26/05 for previous violation of this section. <br />c. The proposed JTD amendment received on August 2"d, 2005 was rejected by this LEA in a response letter dated <br />August 23`d, 2005 due to conflicting with existing approvals for the facility. <br />5. AC T27CCR20580 No light was provided at both active faces. Between 6:00 AM and 6:45 AM, all activities at both <br />active faces were performed in the dark. <br />a. Provide adequate lighting at all active faces. <br />b. Ensure there is adequate lighting at all working area at all time. <br />6. AC T27CCR20690 Tarps that were used as ADC were torn.
