Also see inspection dated 4/20/05, 5/25/05, 6/24/05, 7/27/05, 8/26/05, 9/29/05, 10/28/05 and 11/28/05 for previous <br />violation of this section. <br />c. The proposed 3TD amendment received on August 2nd, 2005 was rejected by this LEA in a response letter dated <br />August 23`d, 2005 due to conflicting with existing approvals for the facility. <br />3. AC T27CCR20590 Drivers do not wear hard hats and/or safety vests when they get out of the vehicle(s) at the active <br />face area. <br />a. Provide adequate safety equipment to personnel who work at or near active face area. <br />b. Ensure that all drivers are wearing safety equipment when they are working outside of the vehicle near active face. <br />4. AC T27CCR20690 The tarp used as ADC has 2 big holes. The waste was exposed in through those holes. <br />a. Repair or replace those tarps. <br />b. A violation will documented if the torn tarp is used as ADC during next inspection.. <br />5. AC T27CCR20919 The shallow probe of GPI I and the shallow and middle probes of GP 12 might be plugged. <br />There was a long air influx into those probes after gas is sampled. <br />a. Provide functional gas probe/sample port at all property boundary. <br />b. Ensure that those probes were not plugged or clogged. Submit finding(s) to EHD. <br />c. Also see inspection report dated 10/28/05 and 11/28/05 for the same concern. <br />6. Violation T27CCR20919.5 The concentrations of methane gas were as following: <br />In GP9: 40.50 % in shallow 37.70 % in middle 0.60% in deep <br />In GP 12: 39.50 % in shallow 22.00 % in middle 18.20 % in deep <br />a. Ensure that the concentration of explosive gas at the property boundary is below 5 % by volume. <br />b. Follow directions listed in T27CCR20919.5(c) and in the Amended Notice and Order 03-01. <br />c. Also see previous inspection report for the previous violation of this section. <br />d. The compliance date for Amended Notice and Order 03-01 was November 20, 2005. The compliance date had <br />passed. The facility is still in violation of T27CCR20919.5 Explosive Gas Control. <br />7.
