Provide justification for such excessive usage of the cover and other beneficial reuse to LEA and the Board. <br />b. Also see inspection dated 4/20/05, 5/25/0, 6/24/05, 7/27/05, 9/29/05, 10/28/05 and 11/28/05 for previous area of <br />concern of this section. <br />8. Notes: <br />A. The C&D waste pile is removed twice per year, according to the operator. <br />B. The extension of the Amended Notice and Order 03-01 expired on November 20, 2005. The compliance date has passed. <br />This facility is still in violation of T27CCR20919.5 Explosive Gas Control. <br />C. There are several erosions and pondings from yesterday rain. Ruben Ramirez, Operation Supervisor, is currently <br />working on repairing and controlling them. <br />D. On December 22, 05, EHD received a complaint regarding heavy mud on Austin Rd in front of the gates of Forward <br />Landfill. On December 23, 05, Mr. Kith and Mr. McClellon of EHD visited the site and confirmed the heavy mud on <br />Austin Rd in front of the gate of Forward Landfill. At the time of the visit, there was also heavy mud on the facility <br />driveways. The street sweeper was not operating, and no one was working on controlling the muddy condition in the <br />facility and on Austin Road. <br />During this inspection, there is only trace amount of mud on the Austin Road and on the facility driveways.
