COMPLAINANT ORDERED <br />CHICKEN CHOW FUN, (NOODLES, CHICKEN AND VEGETABLES), AND CHICKEN FRIED RICE, (CHICKEN, RICE, <br />EGG, AND VEGETABLES.) COMPLAINANT'S DAUGHTER DELIVERED THEM THE ORDER. COMPLAINANT PLACED A <br />SMALL AMOUNT OF THE FOOD IN A BOWL TO CONSUME AND PLACED THE REST IN THE REFRIGERATOR. UPON <br />CONSUMING FOOD, COMPLAINANT BEGAN TO EXPERIENCE PROBLEMS WITH BREATHING AND A LITTLE BIT OF <br />NAUSEA. COMPLAINANT HEATED UP MORE OF THE FOOD LATER IN THE EVENING AND BECAME MORE SICK. <br />COMPLAINANT HEATED UP MORE AFTERWARDS AND WENT INTO WHAT THEY DESCRIBE AS ANAPHYLACTIC <br />SHOCK AND FOOD POISONING. THEY HAD SHORTNESS OF BREATH, NAUSEA, CHILLS, VOMITING, AND <br />DIARRHEA. COMPLAINANT STATES THAT THEY ARE ALLERGIC TO SHELL FISH AND INFORMED THE EMPLOYEE <br />THAT WAS TAKING THE ORDER OF THAT AND TO NOT USE THE SAME UTENSILS AND TO CLEAN GRILL BEFORE <br />PREPARING FOOD ON GRILL THAT SHELL FISH WAS PREPARED ON. COMPLAINANT WENT TO DAMERON <br />EMERGE <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately.
