SAN JOAQUIN <br /> Environmental Health Department <br />COUNTY <br />(Tr t,ivS c owc <br /> Time In: 12:00 pm <br />Time Out: 12:42 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: CHOW TO GO CHINESE RESTAURANT Date: 10/20/2022 <br />Address: 3255 W HAMMER LN, STOCKTON 95209 <br />Owner/Operator: MA, JUN JIE Telephone: (209) 951-5407 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: C00055299 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint. <br />COMPLAINANT ORDERED FOR TAKE OUT ON 10/8/22 SOMETIME IN THE AFTERNOON. COMPLAINANT ORDERED <br />CHICKEN CHOW FUN, (NOODLES, CHICKEN AND VEGETABLES), AND CHICKEN FRIED RICE, (CHICKEN, RICE, <br />EGG, AND VEGETABLES.) COMPLAINANT'S DAUGHTER DELIVERED THEM THE ORDER. COMPLAINANT PLACED A <br />SMALL AMOUNT OF THE FOOD IN A BOWL TO CONSUME AND PLACED THE REST IN THE REFRIGERATOR. UPON <br />CONSUMING FOOD, COMPLAINANT BEGAN TO EXPERIENCE PROBLEMS WITH BREATHING AND A LITTLE BIT OF <br />NAUSEA. COMPLAINANT HEATED UP MORE OF THE FOOD LATER IN THE EVENING AND BECAME MORE SICK. <br />COMPLAINANT HEATED UP MORE AFTERWARDS AND WENT INTO WHAT THEY DESCRIBE AS ANAPHYLACTIC <br />SHOCK AND FOOD POISONING. THEY HAD SHORTNESS OF BREATH, NAUSEA, CHILLS, VOMITING, AND <br />DIARRHEA. COMPLAINANT STATES THAT THEY ARE ALLERGIC TO SHELL FISH AND INFORMED THE EMPLOYEE <br />THAT WAS TAKING THE ORDER OF THAT AND TO NOT USE THE SAME UTENSILS AND TO CLEAN GRILL BEFORE <br />PREPARING FOOD ON GRILL THAT SHELL FISH WAS PREPARED ON. COMPLAINANT WENT TO DAMERON <br />EMERGE <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION -- TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />COMMENTS AND OBSERVATIONS <br />I went to the facility to conduct the complaint investigation. <br />Operator is not aware of the compliant. <br />Operator does not recall being told from a customer that to "NOT USE THE SAME UTENSILS AND TO CLEAN GRILL <br />BEFORE PREPARING FOOD ON GRILL THAT SHELL FISH WAS PREPARED ON" <br />Per operator, the inly seafood prepared at the facility is shrimp. <br />Chow fun is bean sprouts, onion and chicken <br />Fried rice is peas, carrots, rice and onions <br />No egg is in prepared in the fried rice. <br />Chicken , noodles, and broccoli is stored in the upright refrigerator until needed (41F) <br />Broccoli is cut in the morning and placed in the upright refrigerator until needed. <br />Food is cooked when ordered. <br />I observed pork on the drain board of the 3 compartment sink. <br />Per operator pork was just delivered and is going to be prepared and has been out for approximately 20 minutes. <br />Operator placed pork in the upright refrigerator. <br />FA0001882 C00055299 SC544 10/20/2022 <br />EHD 16-23 Rev. 04/07/22 Page 1 of 2 Complaint Inspection Report <br />1868 E.
