Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO6GVRMDI Site Location: 16777 HOWLAND RD <br />Activity Summary <br />Activity Date <br />01/28/2022 I received a voicemail message from Greg Meyer stating that a contractor is on <br />site right now and has started clean-up. I called and left a voicemail message for <br />Greg requesting for an estimated time the excavation will be completed. <br />I received a call back from Greg informing me that they expect to be done <br />excavating around 2-3pm. Greg also stated that he is working on providing <br />additional information regarding the toxicity of the processed water. <br />I received a call from Greg at 3:26pm informing me that they have excavated <br />approximately 4 inches of soil so far and do not plan to backfill because the area is <br />still leveled. Greg stated that the clean-up should be ready for me to verify on <br />Monday. <br />01/31/2022 Inspector Kristina Saetern and I met with Greg Meyer of Simplot Co at the spill <br />location. Per Greg, the impacted soil was excavated on 01/28/2022 and placed <br />into two 20 yard roll-off bins (provided by Ponder Environmental). The clean-up <br />was performed by Rain for Rent. Rain for Rent subcontracted GHD for sampling. <br />The waste will be profiled for proper disposal. <br />I returned to the office and generated an Emergency Response Record (ER01) <br />and site map. Completed photo log. <br />I called Greg Meyer back and asked if the pH of the impacted area was tested <br />during the clean-up. Greg stated they did not test the pH and will provide me with <br />additional information once results are available. Greg provided photos of the pH <br />reading of 3 different areas that showed an approximate pH of 5. <br />02/01/2022 I contacted Greg regarding the clean-up. I informed Greg we would not be able to <br />determine adequacy of the clean-up until we have received additional information <br />regarding the contents of the processed water and it's characteristics. <br />02/10/2022 Reviewed analytical data provided by Meyer, Greg <Greg.Meyer©simplot.com> <br />for the process water. Metals and pH are within non hazardous levels. Called . <br />(209) 858-6470 left a voice message asking if they have a fish bio results on the <br />process water for toxicity. <br />03/01/2022 I returned a call to Greg Meyer. Greg wanted to know what was still needed to <br />close the complaint. I informed Greg that the analytical results and disposal <br />records are still needed to verify proper disposal. Greg stated he would provide a <br />copy of the analytical results either today or tomorrow. <br />03/23/2022 I returned a call to Greg Meyer informing him that we have received the analytical <br />results and disposal proposal for the excavated soil.
