Box 110 <br />Lodi, California 95240 <br />Dear Mr. Sandford: <br />Enclosed are pertinent sections from Title <br />Code, relating to reportable diseases and <br />s <br />Section 2500: designates the reportable diseases and conditions, and A <br />identifies those responsible for reporting. j <br />V� <br />< f <br />N" <br />Section 2502: requires reporting of outbreaks of other infectious or 'M <br />parasitic diseases not specified in Section 2500. <br />Section 2503: requires reporting of cases of unusual diseases not specs <br />f ied in Section 2500. rk <br />,Section 2505: requires notification by laboratories when positive find- <br />{ Ings related to diphtheria, gonorrhea, syphilis, tuberculosis, or typhoi, <br />fever are recorded. <br />The remaining sections of the reporting regulations are included for you <br />:review, particularly in regard to the requirements for handling patients <br />having various reportable diseases. <br />}.'*.Reportable diseasesandconditions should be reported to the Health Dis <br />triet, using the enclosed Confidential Morbidity Report (State Form PM ,n <br />110). A supply of these report forms has been forwarded to you under <br />separate cover. I suggest that a booklet of these forms be placed in th <br />Emergency Room and in all other nursing stations throughout the Hospital <br />Nurses may assist in efforts to improve reporting by placing a blank Con <br />fidential Morbidity Report on any record which includes a diagnosis of <br />reportable disease, thereby encouraging physicians to complete the repor <br />Nurses or other non-medical personnel may actually complete the reports, <br />if records include a clear-cut diagnosis of reportable disease and the Xj <br />attending physician is identified on the report. All Confidential Mori, <br />bidity Reports should be mailed to the San Joaquin Local Health District <br />P.O. Box 200% Stockton, California 95201. <br />Telephone reports are encouraged for those serious comcab <br />ie diseaet <br />listed in Section 2501(c) (cholera, botulism, den ue fever, plague, re- <br />lapsing fever, smallpox, typhus, and yellow fever, and for e10ueral tithe <br />t <br />
