REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. CORRECTIVE ACTION: The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity. ------------------------------------------------------------------------------------ 2)OBSERVATION:The smart sensors were found to be in alarm since[DATE]and actions have not been taken to address the alarm. REGULATION GUIDANCE:All monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to address the alarm under permit and inspection of the EHD. This is a Class II violation. 307 HSC 25290.1(c3), 25290.2(c3) Failure to keep water out of the secondary containment system (post-Jul 2003). OBSERVATION: -Approximately 15 gallons of water was observed in the 87 fill sump. -Approximately 15 gallons of water was observed in the diesel fill sump. -Approximately 2 cups of water was observed in UDC 1/2. REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. CORRECTIVE ACTION: The service technician removed all of liquid from the sumps and returned the sensor to its proper location. Immediately contact a properly licensed, trained, and certified contractor to address the water intrusion into the sumps under permit and inspection of the EHD. This is a repeat violation, Class II.
