0 i
Other-Narrative
The Medical Waste Management Plan on site is not up to date. The MWMP approved by
this Enforcement Agency states waste medication whether partial dose, empty vial or any and
all such waste that contains medication as pharmaceutical waste. Onsite during the
inspection medicine vials were observed in biohazardous sharps containers, pharmaceutical
sharps containers and solid waste containers. Vials in solid waste containers could be
verified as empty vials.
The MWMP also states Environmental Services Technicians are responsible for
collection of medical waste and transport to the designated areas for off-site disposal. On
this date inspection this Enforcement Agency was informed that collection and transport to
the designated areas for offsite disposal is being performed by house keeping services not an
Environmental Technician.
Within 30 days provide to this Environmental Agency for review 1 of 2 things:
1)An updated MWMP describing this facilities method for
segregation storage and transport of pharmaceutical waste:and the
personnel responsible for collecting and transporting medical waste to
the designated accumulation area for off-site treatment. Provided in
the updated Medical Waste Management Plan. Sent with this letter.
2.)Documents showing training given to staff by this facility on
implementing the currently approved medical waste management plan.
All Lodi Memorial Hospital employees at the time of employment go
through an orientation education where the Medical Waste
Management Plan of segregation, collection, storage and transport is
explained. Annually, the hospital provides mandatory refresher
training and the Medical Waste Management Plan is one segment of
this training. There is also department specific training that takes
place which defines the roles and responsibilities, segregation rules,
storage and transfer of each type of medical waste stream through out
the facilities. All training has been documented by the hospital
education department.
—► The primary goal training must achieve is a uniform method for the
segregation, collection, storage and transport of medical waste stated
in this Large Quantity Generator's Medical Waste Management Plan.
Please let me know if you require any additional information.
Sincerely,
Gayle Moses, HEM
Safety Security Manager
Lodi Memorial Hospital
CC: Tak Saito
Facilities Management Director
4
