Lower Sacramento, Lodi CA 95241
o HSC 118280(b)—Medical Waste Label and Container. At the West facility,
Biohazardous bags were observed on the ground stored in the designated storage
accumulation area. During the inspection the Safety Security Manager placed the
Red Biohazardous bags in a Biohazardous waste container. To containerize
biohazard bags place them for storage, handling,or transport in a rigid container. The
container must be labeled with the word"Biohazardous Waste"and the international
symbol or the word"biohazardous Waste"on the lid and sides so as to be visible
from any lateral direction.
■ Education with the Environmental Services staff regarding the appropriate
placement and disposal and labeling methods of red bag waste has been
conducted on Nov. 6,2013 for the 800 S. Lower Sacramento Road location.
The training records are attached.
o HSC 118280(d)(1)(A)—Medical Waste must have pick-ups performed at least once
every 7 days. A period of 10 days elapsed between pick-ups at the Lodi Health West
facility. A pick-up was performed on 12-18-12 and the next pick-tip was on 12-28-
12. The requirement is that biohazard waste must be picked-up at least once every 7
days.
■ The waste was not picked-up by Stericycle on 12-21-13. We have
communicated with our vendor that we require pick-tips every 7 days.
Please let me know if you require any additional information.
Sincerely,
Gayle Moses,HEM
Safety Officer/Safety Security Manager
Lodi Memorial Hospital
CC: Chris Dickey
Facilities Management Director
