SAN JOAQUIN <br /> Environmental Health Department <br />I-11Y <br />Time In: 2:25 pm <br />Time Out: 3:04 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: 99 CENTS ONLY STORES #157 Date: 10/12/2022 <br />Address: 300 W KETTLEMAN LN, LODI 95240 <br />Owner/Operator: Telephone: (209) 339-9103 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: C00055277 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />HUGE RATS INSIDE STORE***PLEASE SEND INSPECTOR AS SOON AS POSSIBLE*** <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />COMMENTS AND OBSERVATIONS <br />I spoke to the manager who was aware of the compliant prior to this investigation. <br />Per the manager rodent traps were placed in the back receiving area last night and 2 rodents were caught. <br />I observed fresh rodent droppings on the shelf in aisle #1 where dry packaged pasta shells are displayed. <br />Clean and sanitize all shelves and affected areas. <br />I observed debris and accumulation under pallets that hold soda. Clean and sanitize area. <br />Observed water accumulation in the floor of the back receiving area near the walk-in. Clean. <br />Observed roll up door in the back receiving area. Sea Vweatherproof gaps that are above roll up door to prevent possible <br />rodent entrance. <br />Operator to provide effective pest control. <br />A chargeable follow up inspection to occur on or after 10/19/2022 <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
