Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: CO5BHIC6N Site Location: NORTH BOUND HVVY 99 BEFORE E WOODBRIDGE RD EXIT <br />Activity Summary <br />Activity Date Recorded by <br />02 /24/2022 Drafted notice to abate. Emailed to Richard Thai, Operations with Obaba PRESTO <br />International Corporation (richard@obabaseafood.com). <br />Received text from Robert Shanks, CalTrans asking for the Cal OES number. <br />Text him back the number and he let me know that the clean up is scheduled for <br />3/3/2022 at 9:00am. <br />02/28/2022 Received voicemail from Ray Henry of RAH Environmental. They are planning PRESTO <br />clean up on Thursday but the weather is expecting rain so he will call me back on <br />Wednesday morning to let me know if the clean up will still happen on Thursday. <br />03/01/2022 Printed out 5104, photo logs, emergency response record, site map, emails, PRESTO <br />complaint history, notice to abate and emails, copied business cards. Put together <br />in packet. <br />03/01/2022 Printed documents for complaint packet PRESTO <br />03/02/2022 Called Ray Henry and left a voicemail to follow up on clean up that was tentatively PRESTO <br />scheduled for tomorrow morning. Received a call back that they are now planning <br />for 3/9/2022 to do the clean up. <br />03/03/2022 Robert Shanks with CalTrans notified that clean up is scheduled for 3/8/2022 at PRESTO <br />9:00am <br />03/08/2022 Arrived on site and met with Juan Arnnendarez, Caltrans HazMat Manager. PRESTO <br />Observed clean up completion. Approximately 1 cubic yards of waste was <br />generated. PPM measurements were taken from the excavated area, 2.0ppm, 2.0 <br />ppm and 0.7ppm were the results. No staining was observed in the excavated <br />area. Clean up is complete. <br />03/10/2022 Time stamped photos. Moved to P drive. PRESTO <br />03/28/2022 Received manifest for excavated soil and sample results from Raymond Henry Jr., PRESTO <br />President for RAH Environmental, ray@rahenv.com. 2 cubic yards of non-RCRA <br />hazardous waste solids were transported to US Ecology. Ray mentioned that <br />negligible results of xylene were detected and asked if EHD requires additional soil <br />excavation. Reviewed sample results, all show non-detect. Discussed with lead <br />senior. Emailed him back that the results all show non-detect so based on that, no <br />extra excavating would be required but asked him if he had any concerns <br />regrading xylene and if that is why he was asking about it. <br />03/29/2022 Received email from Ray Henry that he wanted to bring up the diesel and motor oil PRESTO <br />detected amounts, but if the county is okay with the results he is okay. Called lead <br />senior for assistance. Called Juan Armendarez, CalTrans who was copied on all <br />the emails from Ray. Since CalTrans is the property owner, checked with Juan to <br />see his thoughts on the results. Per Juan, he is okay with the results. Emailed Ray <br />back that after discussing with CalTrans, the results are okay. <br />09/14/2022 Generated photo log for clean up.
