Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COMRKBCLB Site Location: CORNER OF W HOWARD ROAD AND CROCKER ROARD <br />Activity Summary <br />Activity Date <br />05/26/2022 <br />Recorded by <br />Received the page from 209-468-6000 at 7:56 AM and made phone call. Obtained SAETERN <br />contact information. Phoned Sheila of California Highway Patrol (209-943-8600). <br />Per Sheila, there was a vehicle accident on S. Howard Road and Crocker Road <br />that caused the release. Approximately 5 gallon of diesel and hydraulic fluid <br />mixture onto agricultural soil. Clean up is in progress through San Joaquin County <br />Road Maintenance. Absorbents have been placed onto the road. <br />Obtained Sargent Kyle Plant number and called him. Per Sgt Plant, the clean up <br />will be conducted by All Environmental Services, approximately 45 gallons of <br />diesel was released onto the road and the driver drove onto the property releasing <br />5 gallons onto soil. The spill have been contained. Per Sgt Plant, the spill on the <br />road was being clean up by San Joaquin Public Work and the soil will be handled <br />by All Environmental Services. Requested for contact information of All <br />Environmental Services, requested was denied due to Sgt do not have the <br />information. <br />Discussed with M. Henry, Program Coordinator. <br />Phoned Road Maintenance, spoke to Ted Nishimura, who confirmed they are not <br />conducting the clean up and All Environmental Services will be the clean up <br />company. <br />Arrived on site, met with Sargent Kyle Plant of CHP. Approximately 45 gallons of <br />diesel spilled onto the asphalt and 5 gallon of diesel spilled onto soil due to a <br />vehicle accident. Spill is contained with absorbent powder. No waterways or storm <br />drains impacted. Spill cover an approximate area of 100x75ft.The asphalt of the <br />road have been cleaned by All Environmental Services LLC and placed into two 55 <br />gallon drums. A black plastic tarp was placed onto the soil right off the road. A soil <br />sample was taken from the site of impact for analysis. Clean up will be revisited <br />and conducted by All Environmental Services and waste will be disposed as <br />hazardous waste. Final clean up is pending USA Dig. <br />All Environmental Services to notify EHD when clean up is complete and prior to <br />any backfilling for verification. All Environmental Services to provide analytical <br />results and final disposal records for waste generated from the clean-up. <br />Spoke to Camren Herrin of J&E Truck Service and Repair, who stated the saddle <br />tank can hold up to 75 gallons of diesel but he was unsure of the amount. The <br />responsible party and All Environmental Services were not on site at the time of <br />arrival. Automobile was removed and there was no casualty. Obtained responsible <br />party information, Coby Kalar, business owner of Kalar Trucking and All <br />Environmental Services - Brandon Espinoza's phone number. <br />Phoned Brandon Espinoza of All Environmental Services 209-648-8060. <br />Estimated time of arrival was 20 minutes. <br />Updated M. Henry. <br />All Environmental Services arrived around 9:20 AM. Per Brandon, he will be <br />USA-Dig the soil and more absorbent will be placed onto the road to be brushed. <br />Black plastic tarps was placed over three sections of the soil. Samples of <br />contaminated soil was observed taken with a bottle by Brandon Espinoza. <br />Measurements of the spill area for USA Dig. Spill cover an approximate area of <br />100x75ft. Requested Brandon to place more absorbent to clean the road. All <br />absorbent was placed into two 55 gallon drums. A hazardous waste sticker was <br />placed onto both drums, photos was obtained. Per Brandon, the manifest is not <br />fully filled out because he does not know where to take the drums and it will stay <br />on his trailer until he obtained the soil sample analytics result. <br />5105 rpt <br />Page 2 of 4
