Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COUDUOEAJ Site Location: W CHARTER WAY & S FRESNO AVE <br />Activity Summary <br />Activity Date Recorded by <br />7:55 PM Arrived on site. <br />Upon arrival, met with Diego Milian, Supervisor of All Environmental Services, <br />Officer Camarena, and Rodney Sok, truck driver of MSG3 Logistics Inc. Obtained <br />informations of each personnel. Per Rodney Sok, truck driver, the saddle tank was <br />side swiped by a car that was being chase for a hit and run. Observed absorbents <br />placed onto the manhole, gutter covering an area of 100 ft and middle turning lane <br />of W Charter Way. Soft boom was observed at the entrance of the storm catch <br />basin which was placed by the fire department. Diesel from the crushed saddle <br />tank was observed leaking onto the absorbent. <br />Upon arrival of Richard Stiffler, the impacted man hole was opened to be <br />inspection. No diesel was observed inside the man hole. The impacted storm <br />catch basin was opened to be inspected. A cloth was observed at the entrance <br />that absorbed the diesel. No diesel was found inside the storm catch basin and <br />adjacent man hole. <br />Updated E. Manzo. <br />Generated official inspection report. <br />Phoned Marchin, Owner of MSG3 Logistics Inc, 847-668-7388. Could not leave <br />message. <br />Received return phone call from Ralph Gollonk, 224-400-9009 dispatch for MSG3 <br />Logistics Inc, confirmed they have been notified and provide contact information. <br />msg3log isticsinc@gmail.com. <br />Clean up was completed by All Environmental Services. Three 55 gallon drums of <br />waste was generated from clean-up to be disposed as hazardous waste. <br />9:45 Left site. 10:15 arrived. <br />10:35 E-mailed official inspection report for the hazardous waste clean-up to <br />msg3logisticsinc@gmail.com. <br />08/15/2022 Researched property owner to inform them of the incident that had occurred on SAETERN <br />8/12/2022. Phoned Moss Gardens Owners' Association, (209)-464-4413, could <br />not leave a voice message for notification. <br />Phoned Richard Stiffler, City of Stockton Storm, 209-993-3274 to request for <br />information on the privately own pipe on Charter Way. Per Richard, the line shows <br />as privately own on their map and the privately own line can be the homeowner <br />association or individually own." <br />08/17/2022 Generated photolog.
