Within thirty-five (35) days of the removal of medical waste from our premises, the destination facility's owner/operator must provide this facility with a completed copy of the tracking form,which includes the following: a. The method of treatment of the medical waste received from this facility; b. The location of the landfill used to deposit our medical waste;and c. The date and signature of the destination facility's owner/operator. 35-Day Limitation 8. If, after thirty-five (35) days, our facility has not received the signed and dated tracking form.the Infection Control Coordinator(or designee)will try to determine the location of the medical waste. Documentation will be maintained of such attempts and filed in the business office. Filing an Exception Report 9. if, after forty-five (45) days, a signed and dated tracking farm has not been received, the Administrator will file an Exception Report with the state and the EPA Regional Administrator the next day. Contents of]Exception 10. Exception Reports shall include at least: Report a_ A letter explaining our efforts to locate the waste and the results of such effects; and b. A legible copy of the original tracking form. Maintaining Documentation 11.. The administration shall maintain copies of all tracking formas, shipping logs, exception reports, etc., for at least three (3) years.from the date of receipt or as required by current federal,state,or Local statutes.
