I have tried unsuccessfully to <br /> get a hold of you on the phone regarding corrections to the Medical Waste Management Plan (MWMP). So the <br /> following are the corrections to the MWMP I would like implemented and resubmitted by November 12, 2010: <br /> 1. The MWMP needs to have the name of the facility, address, type of business(nursing facility), estimated <br /> monthly quantity of medical waste generated (can call Stericycle for this information), name and business <br /> address of the registered hazardous waste hauler(Stericycle), name and address of the offsite medical <br /> waste treatment facility to which the medical waste is being hauled (can call Stericycle for this information), <br /> an emergency action plan (procedure if Stericycle is unable to pickup the medical waste), and a statement <br /> certifying that the information in the MWMP is complete and accurate. <br /> 2. The MWMP needs to state that there is no onsite treatment of the medical waste, that all biohazardous <br /> waste containers need to have appropriate labels ("Biohazardous waste"or"Biohazard" and the biohazard <br /> symbol)on the lid and sides, and that medical waste will not be stored on the premises for more than 7 <br /> days(page 26, #9). <br /> 3. The MWMP needs to state that spills, contaminated items, and medical waste containers that are <br /> decontaminated with <br /> bleach, are done so with a minimum concentration of 500ppm available chlorine. <br /> If you have any questions you can call me at(209)468-3178 or email me at bescotto sicehd.com.
