I have tried unsuccessfully to
get a hold of you on the phone regarding corrections to the Medical Waste Management Plan (MWMP). So the
following are the corrections to the MWMP I would like implemented and resubmitted by November 12, 2010:
1. The MWMP needs to have the name of the facility, address, type of business(nursing facility), estimated
monthly quantity of medical waste generated (can call Stericycle for this information), name and business
address of the registered hazardous waste hauler(Stericycle), name and address of the offsite medical
waste treatment facility to which the medical waste is being hauled (can call Stericycle for this information),
an emergency action plan (procedure if Stericycle is unable to pickup the medical waste), and a statement
certifying that the information in the MWMP is complete and accurate.
1. The MWMP needs to state that there is no onsite treatment of the medical waste, that all biohazardous
waste containers need to have appropriate labels("Biohazardous waste" or"Biohazard" and the biohazard
symbol)on the lid and sides, and that medical waste will not be stored on the premises for more than 7
days(page 26,#9).
1. The MWMP needs to state that spills, contaminated items, and medical waste containers that are
decontaminated with
bleach, are done so with a minimum concentration of 500ppm available chlorine.
If you have any questions you can call me at(209)468-3178 or email me at bescotto .sjcehd.com. Thank you.
Ben Escotto
REHS
10/28/2010
