<br />OBSERVATIONS: Hand sink was observed unsecured to wall. Re-secure hand sink to wall and replace caulk within two
<br />weeks.
<br />CALCODE DESCRIPTION: The potable water supply shall be protected with a backflow or back siphonage protection device, as required
<br />by applicable plumbing codes. (114192) All plumbing and plumbing fixtures shall be installed in compliance with local plumbing
<br />ordinances, shall be maintained so as to prevent any contamination, and shall be kept clean, fully operative, and in good repair. Any hose
<br />used for conveying potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. (114171,

<br /> #45 Floors, Walls, Ceilings; Clean and Maintained
<br />OBSERVATIONS: Floors, walls and ceilings were observed with heavy debris and build-up. The following was observed:
<br />-Ceiling vent and surrounding area near the hood was observed with heavy dust accumulation
<br />-Walk-in freezer floor was observed with ice accumulations and heavy food debris
<br />-Heavy build-up observed on hard to reach floors under hand sink, table with tea brewers, under the three compartment sink
<br />and cooking equipment under hood
<br />-Heavy build-up was observed on walls below hand sink, above and below three compartment sink and walls surrounding
<br />cooking equipment
<br />-A lot of water was observed under the table that holds the tea brewers. Clean up water and fix any leaks if necessary
<br />Detail clean all the above areas within one week. Maintain clean. (REPEAT)
<br />Broken base tiles were observed at the mop sink. Repair broken tiles within two weeks so that base is smooth, cleanable
<br />and non-absorbent.
<br />The base at the outside of the walk-in entrance was observed damaged. Repair base so that it is smooth, cleanable and
<br />non-absorbent within three weeks.
<br />CALCODE DESCRIPTION: The walls / ceilings shall have durable, smooth, nonabsorbent, light-colored, and washable surfaces. All floor
<br />surfaces, other than the customer service areas, shall be approved, smooth, durable and made of nonabsorbent material that is easily
<br />cleanable. Approved base coving shall be provided in all areas, except customer service areas and where food is stored in original
<br />unopened containers. Food facilities shall be fully enclosed. All food facilities shall be kept clean and in good repair. (114143 (d),
