Hunt to conduct
cleanup. Patriot Supervisor Damien Webb was in charge of the cleanup. Per
Damien, the chemical (Concentrate EVP) had released while the truck driver was
driving. It is unknown how far the truck driver drove with the leaking tote. Per
Patriot, stained asphalt was observed approximately 1/4 mile from the site of where
the driver had parked.
Patriot used absorbent to clean the heavily impacted areas of the released
chemical. They also used absorbent to clean the trailer. 50 gallons of liquid were
removed from the leaking tote and placed into a 55 gallon drum. The absorbent,
tote, plastic tarp, and excess liquid were disposed of as non-RCRA hazardous
waste. Five 55 gallon drums of solid waste and one 55 gallon drum of liquid waste
was generated from the cleanup. Observed final cleanup of the heavily
contaminated areas. The other lighter stained areas of the asphalt (1/4 mile) were
not cleaned. Per Patriot, the area would need to be power washed to remove the
staining. The chemical had dried and was not able to be cleaned up through
absorbent. Patriot and Heather Byrne, Premium Environmental Services had no
concerns of further remediation being required. An inspection report was
generated and emailed to Heather at
heather@premiumenvironmentalservices.com.
Activity Date Recorded by
11/15/2022 Called Heather Bryne, 812-853-9400 and informed her I was going to the site to ALANIZ
verify cleanup.
ALANIZ
5105.rpt Page 1 of 3
