Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00055093 Site Location: E HAZELTON AVE & AURORA ST <br />Activity Summary <br />Activity Date <br />08/03/2022 Spoke with Robert Isom of Patriot Environmental. Robert stated that Patriot <br />Environmental was on site Tuesday (8/2) night. He provided me with Matt Musoffs <br />contact information (916-347-8717) since Matt was the person on site conducting <br />clean up. I called Matt who stated they were on site last night and per Matt they did <br />not secure the area. They were on site today (8/3/22) to conduct clean up and <br />stated they completed clean up by around 9:30am. I was not contacted to observe <br />clean up and per Matt they had left the area already. Received pictures and exact <br />location from Matt via text. Per Matt, waste was going to be hauled off as <br />non-hazardous to the Yolo Landfill because the contents were acrylic and latex <br />and they were dry making them non-hazardous. Per Matt, waste was collected in a <br />20-yard roll off bin. Per Matt, no sampling was conducted. I called Michael Algots <br />with UPRR and informed him what Matt Musoff had told me. Michael stated he <br />would speak with Matt. Received a call back from Michael apologizing and stating <br />that he had notified Patriot to secure the area, that they contact me when clean up <br />is being conducted and make a hazardous waste determination. Michael stated <br />that he would have Patriot collect samples and make a hazardous waste <br />determination. Per Michael, the roll off bin will be brought back to the site where <br />contents were collected, near Mormon Slough. Per Matt, roll off bin contains four <br />locks and is secured. <br />Called Matt inquiring whether the roll off bin had been brought back to the site. Per <br />Matt, a 1/2 gallon ziploc bag of soil sample was collected and will be sent off to <br />Pace Labs for a hazardous waste determination. Received a copy of the chain of <br />custody from Matt via text. <br />Recorded by <br />GARCIA-MEJIA <br />09/13/2022 Contacted Michael Algots regarding the status of the samples and if they received GARCIA-MEJIA <br />final results from the lab. <br />09/15/2022 Reviewed analytical results via email from Ambrose Kong GARCIA-MEJIA <br /><Ambrose.Kong@patriotenvironmental.com>. No hazardous values were <br />detected. Replied to email thread asking whether the waste has already been <br />disposed of and if so how? Ambrose replied to my email stating that the waste has <br />not yet been disposed of but the plan is to dispose of it as non-hazardous at the <br />Potrero Hills Landfill. <br />10/25/2022 Emailed Ambrose Kong with Patriot Environmental to follow up my email on GARCIA-MEJIA <br />9/15/22 to see if they have disposed of the waste. <br />12/01/2022 Waste was sheduled to be disposed of on 11/4/22. I emailed Ambrose Kong to GARCIA-MEJIA <br />check in and see if it was taken to the landfill as scheduled and reminded him to <br />please provide me with a copy of the final disposal record. <br />Received copy of the signed non-hazardous waste manifest as well as the Potrero <br />Hills Landfill weight ticket via email from Ambrose Kong <br /><Ambrose.Kong@patriotenvironnnental.com>. Waste was disposed of at the <br />landfill on 10/29/22. Saved email and waste records to the file. <br />12/05/2022 Printed out all necessary forms and abated the complaint.
