Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COATV9SDZ Site Location: 17151 S MCKINLEY AVE <br />Activity Summary <br />Activity Date Recorded by <br />05/03/2022 Went to site to verify clean-up with Gabby Garcia-Mejia (EHS). Arrived on site and BAKER <br />met with Greg Meyer (Simplot). Entire spill area has been excavated <br />approximately 4 inches deep. Fifty-eight 55 gallon drums of contaminated soil <br />was generated from the clean-up. Permission to take a sample was granted by <br />Greg Meyer (Simplot). A split sample was requested. Sample was taken <br />approximately 6 feet to the west of where the pipe broke in the soil. Soil was <br />sandy. Two 32 oz glass jars of soil were collected. Greg Meyer picked which <br />sample he would like. Samples were taken at 9:06 am and labeled as soil-la and <br />soil-1b. Greg Meyer took soil-1b. Samples were labeled and sealed with <br />evidence tape. Chain of custody was filled out. Sample was transported in ice <br />chest and placed in sample refrigerator upon return to office. <br />05/03/2022 Generated a photo log of the photographs taken during the final clean up and GARCIA-MEJIA <br />sampling visit. <br />05/04/2022 Took sample to Geoanalytical Laboratory in Turlock. BAKER <br />06/27/2022 Called Greg Meyer (Simplot) at 209-401-9845 and left message for final copies of BAKER <br />disposal records. <br />07/01/2022 Reviewed and summarized analytical. BAKER <br />Sample Soil 1A, dated 5/5/2022 <br />96 hour fish bioassy (mg/L) = 750 <br />Results are below the hazardous waste regulatory level. <br />09/08/2022 Found final disposal records on HVVTS. Waste disposed of on manifests <br />016504199FLE and 016504209FLE dated 7/22/2022 & 7/25/2022. A total of <br />31000 pounds of waste was disposed of. <br />Abated complaint as 01-Field Response-Violations Cited and Corrected. Turned <br />in file to Lead Sr. REHS for review. <br />BAKER <br />5105. rpt Page 2 of 2
