Complaint Investigation Summary Report #r 5105 <br />COMPLAINT ID: COOTAH6UF Site Location: 6540 S AUSTIN RD, STOCKTON, CA 95215 <br />Activity Summary <br />Activity Date Recorded by <br />05/09/2022 Emailed John Brainard again to update the spill report with the new release PRESTO <br />quantity. Emailed Carlos Sanchez with US Ecology to request clean up information <br />and a copy of the manifest. <br />Received an email from John Brainard with a BNSF Railway Cargo Report. <br />Responsible party of Greenwood Motor Lines. The release was suspected to be at <br />a rate of 2 drops per minute. <br />05/10/2022 Received response from Carlos Sanchez: Generator of waste was R+ L Carriers. <br />pH of floor post clean up was approximately 6-7. There was seven 55-gallon <br />drums containing used PPE, plastic and debris waste and one poly tote with <br />aproximately 225g of sodium hydroxide/potassium hydroxide mixture. The waste <br />is still on site pending pick up and disposal this week. He will provide a copy of the <br />manifest after it has been picked up. <br />PRESTO <br />06/28/2022 Entered responsible party information and emailed accounting back that the PRESTO <br />information has been entered. <br />07/26/2022 Received phone call from Larry Musselman requesting information on cargo so he PRESTO <br />can bill appropriately. Gave him some information on the BNSF report. Provided <br />him with John Brainard's name, per Larry he knows John and can contact him. <br />Larry asked if I could email him the report. I told him I will check that BNSF doesn't <br />have it marked as confidential and if it isn't I will forward to him. Found report, did <br />not see any confidential marks, emailed to Larry. <br />09/20/2022 Reviewed packet. Emailed Carlos Sanchez with US Ecology for copy of disposal PRESTO <br />records. <br />09/21/2022 Emailed Carlos Sanchez, Regional Emergency Response Manager with US PRESTO <br />Ecology for copy of disposal records <br />09/22/2022 Received manifest from Carlos. Printed and placed in packet. PRESTO <br />11/04/2022 Everything already printed out for packet except for log2 and 5105. Printed for PRESTO <br />abatement. <br />12/21/2022 Received phone call from Larry Musselman, RL Carriers asking for a unit number PRESTO <br />for his billing.
