Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COVWLHVCM Site Location: 5100 W HVVY 12 <br />Activity Summary <br />Activity Date Recorded by <br />02/24/2023 Received email from Hugh Galyean with a summary of the solid and liquid wastes: PRESTO <br />"Upon receipt of the assignment, we received and reviewed the All Environmental <br />Services, LLC (AES) invoice for their response and cleanup of the spill. We <br />contacted AES who confirmed that the insured contacted AES and requested their <br />response. We requested and obtained supporting documentation for their invoice <br />from AES. According to AES, they responded on the day following the incident to <br />find that an unspecified volume of both diesel fuel and engine fluids had impacted <br />the soil, due to the fire-fighting efforts by the responding Fire Department, and <br />spread across an area measuring approximately fifty (50) feet wide, by eighty (80) <br />feet long. AES pumped the remaining virgin diesel fuel from both burned saddle <br />tanks into two (2) 55-gallon drums, in order to prevent additional spilling of diesel <br />fuel during the future towing and recovery of the remains of the insured vehicle <br />from the scene. The drums were transported back to the AES shop for staging <br />and profiling, prior to disposal. The petroleum-impacted soil was then sampled for <br />analysis and preparation for disposal, prior to a return for excavation. <br />On October 6, 2022, AES returned to the scene with heavy equipment and <br />excavated nearly twenty-eight (28) tons of petroleum-contaminated soil, which was <br />loaded into two (2) roll-off containers. Surrounding, native soil was backfilled into <br />the excavated area, then compacted and regraded to pre-spill contours. The <br />excavated material was transported back to the AES shop for staging and <br />profiling, prior to disposal." <br />Received contact information for All Environmental Services to request copies of <br />the bill of lading and to reference invoice #1310 or Turh Trucking. <br />Emailed Brandon Espinosa, allenvironmentalserviceslIc@gmail.com, to request <br />clearer copy of bill of lad ings and for the bill of lad ings that show the landfill <br />received it. <br />03/01/2023 Received email on 2/28/2023 from Brandon Espinoza of All Environmental with a PRESTO <br />Google link to photos for the incident. He stated that the signature from the landfill <br />is on the bottom. Saved all photos into shared drive. Printed all photos for <br />complaint packet. Printed Brandon's email for complaint packet.
