Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COF49CV7Q Site Location: 3331 CLEMENT AVE <br />Activity Summary <br />Activity Date Recorded by <br />05/11/2022 Called Jacqueline Chan, CDPH Environmental and Occupational Duty officer, left MANZO <br />a voice message with the status update indicating that it's unclear which house is <br />causing the odor and that the complaint was referred to county code enforcement <br />and sheriffs for a team visit for further follow up; left my contact information. Due <br />to the lack of information on the location of the house which is causing the odor <br />this complaint is being abated, a new complaint will be generated if the correct <br />address is received. <br />05/19/2022 Review complaint per E. Manzo, Program Coordinator. Check in with L. Baker, SAETERN <br />inspector, who went out to the property. Residents were recommended to seek <br />medical attention, per residents they have been to the Emergency Room three <br />times. <br />51 05. rpt Page 2 of 2
