Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COGCKSBAT Site Location: 25501 GATEWAY BLVD <br />Activity Summary <br />Activity Date <br /> Recorded by <br />03/29/2022 (5pm-7:18pm), (8:24pm - 9:30pm) <br /> VAN G-LEE <br />(5pm-7:18pm) <br />I arrived on site and met with Derek Collins (Fleet Supervisor) of Costco, Adam <br />Costa (lead driver) of Certified Freight Logistics, and Camren Herrin of J&E Truck <br />Service. Absorbent sand and booms were applied to the impacted area and were <br />containerized in two 55 gallon drums. The release impacted asphalt only. No <br />waterways were impacted, there were no exposures, and no evacuations. <br />Clean-up is incomplete at this time. <br />Initially, Adam stated that Ramos Pressure Wash would be completing additional <br />clean-up by pressure washing the impacted area. Ramos Pressure Wash left the <br />site without completing any additional clean-up. Adam and Camren then informed <br />me All Environmental would be coming out to complete clean-up tonight and <br />would arrive on site in approximately 90 minutes. I informed Adam I would need <br />to verify the final clean-up and requested that I be notified approximately <br />30minutes before clean-up is complete. <br />(8:24pm - 9:30pm) <br />I contacted Brandon Espinoza of All Environmental at 209-648-8060 who informed <br />me that Javier will contact me when they are ready. I contacted Mark Zarate <br />(General Manager) of Certified Freight Logistics at 805-310-7648 to get contact <br />information. Per Mark, he was unaware that All Environmental has been hired for <br />the clean-up. I informed Mark I was informed by Adam and Camren during my <br />visit that All Environmental was contracted for the final clean-up. Mark stated he <br />did not hire them. I provided Mark with Brandon Espinoza's (of All Environmental) <br />contact information. <br />I received a call from Javier of All Environmental informing me that they are almost <br />done with the clean-up. Per Javier, all they did was additional sweeping and <br />picked-up the drums of absorbent and booms. I updated Mark Zarate regarding <br />the clean-up and issued a notice to abate. <br />I received a call from Brandon Espinoza of All Environmental who wanted to <br />confirm if the clean-up was adequate. I informed Brandon I did not go back to the <br />site to verify clean-up because additional clean-up (pressure washing) was not <br />performed as I was previously told. I informed Brandon I will talk to Mark Zarate <br />tomorrow morning and update him once I get clarification regarding their plan of <br />action. <br />03/29/2022 I received spill report 22-1780 at 4:29pm. I generated a complaint record and <br /> <br />VAN G-LEE <br />contacted Derek Collins (Fleet Supervisor) of Costco at 209-839-2586. Derek <br />confirmed there was a 50 gallon release of diesel and that clean-up was in <br />progress. <br />03/30/2022 Provided spill clean up assistance to V. Vang-Lee with EHD concerning hazardous MANZO <br />waste transport without a manifest. <br />03/30/2022 I received photos of the final clean-up from Judy Ramos. I called Judy to request VANG-LEE <br />for a photo of the manifest but she stated she did not have one. I informed Judy <br />the waste cannot be transported without a manifest. Per Judy, approximately <br />20-30 gallons of rinseate was generated and left on site. Updated lead Elena <br />Manzo. <br />03/30/2022 Updated lead regarding complaint status.
