Generated emergency response record and site map. VANG-LEE <br />Contacted Derek Collins of Costco to get clarification regarding the clean-up to <br />see if pressure washing was still going to take place as previously discussed. <br />Derek stated that was what he had requested from the responsible party. I <br />provided Derek with Mark Zarate of CFL's phone number. <br />I contacted Mark and asked for clarification regarding the clean-up. Per Mark, he <br />is unsure at this time if it is needed. I asked Mark if he needed the contact <br />information for Costco and he stated he already had the contact information. I <br />informed Mark I have been communicating with Derek Collins. Mark stated he will <br />update me once they have decided on their next plan of action. <br />I received a call back from Mark stating that Ramos Pressure Wash will be on site <br />at 3pm today to wash and vacuum the impacted area. I contacted Judy Ramos <br />(owner of Ramos Pressure Wash) at 209-200-2174. Judy informed me that the <br />clean-up may take 3 to 4 hours. Per Judy, the rinseate will be disposed as <br />hazardous waste at City of Elk Grove Special Waste Collection. I asked Judy to <br />send me photos of the final clean-up and a photo of the manifest. <br />03/31/2022 I updated lead Elena Manzo regarding the complaint status. I contacted Mark <br />Zarate to discuss manifesting and transporting requirements. I received an email <br />from Mark stating that all waste have been transported to "THE NEAREST <br />AUTHORIZED DISPOSAL FACILITY". I responded to Mark informing him that I <br />will need copies of the final manifests before I could close out the complaint <br />record. <br />VANG-LEE <br />04/08/2022 Completed photo log and printed photos received from PSC and Ramos Pressure VANG-LEE <br />Wash of final clean-up. <br />04/12/2022 I received a copy of manifest 016996500FLE via email from Brian Espinoza of All VANG-LEE <br />Environmental. The manifest is for absorbent and rinseate. I still need the final <br />copy before abating the complaint. <br />05/17/2022 Generated the complaint investigation form (5104), complaint investigation VAN G-LEE <br />summary (5105), daily time and activity summary (Log2). Reviewed daily <br />narratives and organized complaint documents. Abated complaint. Submitted file <br />for final review. <br />5105 rpt <br /> Page 3 of 3
