A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br />client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. HSC 119315(f) — The record of purchase and sterilization certificate were not available for <br />Andrew Soldana's Cheyenne cartridges with lot numbers 2020-51-035 and 2021-22-056. Provide EHD with sterilization <br />certificates for the cartridges mentioned above. <br />11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use <br />paper towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) — A <br />wall -mounted, touchless paper towel dispenser was not observed at the procedure area sink. Install a wall -mounted, <br />touchless paper towel dispenser at the procedure area sink. <br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available for inspection. HSC 119303 — The consent form and medical questionnaire did not include all of the information <br />listed on Form B. Include the following information on the consent form and medical questionnaire and provide a copy <br />to the EHD: <br />Consent form <br />- Description of what the client should expect following the procedure. <br />- Notice that inks are not FDA approved and health consequences are unknown. <br />Medical Questionnaire <br />-Other risk factors for blood borne pathogens <br />31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC <br />119314 — The restroom did not consist of a single -use paper towel dispenser that is wall -mounted and touchless. Install <br />a single -use wall -mounted, touchless paper towel dispenser. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 — There are gaps between the floor and the <br />baseboard. The electrical outlet next to the storage room door is not fixed to the wall. The garbage can in the procedure <br />area was not lined. There is a mail drop in the waiting area that is in despair. Seal the gapped areas between the floor <br />and baseboard throughout the facility, repair the electrical outlet next to the storage room door, ensure the garbage can <br />in the procedure area is lined and block/ close the mail drop. <br />Reinspection on/about: A reinspection fee of $152 per hour may be charged.
