Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: COSH7JAH5 Site Location: 11612 N PEARSON RD, LODI - UNION PACIFIC RAILROAD <br />Activity Summary <br />Activity Date Recorded by <br />07/20/2022 Created complaint. Called to get information in writing that the release was only PRESTO <br />soybean oil. Was not provided a report number initially but was provided one this <br />time for any future calls regarding this incident - #2022-07-19-176-JJL. Received <br />contact information for a hazardous materials manager, Damian Guy. Called <br />Damian to get information on the incident. Per Damian upon arrival they did not <br />observe any odors. They checked all the railcars and all cars were secured. The <br />soybean railcar was the only car that had residue on the side. He said it was <br />approximately 15-gallons of soybean oil that released on the side of the car due to <br />loading error but none released as a result of transportation. The soybean oil was <br />contained in the railcar and did not escape to the ground. They broke the man way <br />seal off to verify nothing else was leaking, nothing was found and they replaced <br />the many way seal. Asked him about the odor and he stated perhaps the golfer <br />smelled something else and assumed it was the railcar since it was near by. <br />Received an email from Damian stating it was soybean oil, no hazardous <br />materials were released and no waterways were impacted. <br />Complaint abated as unable to verify. <br />10/31/2022 Received voicemail transferred from Accounting on a Friday off from Anna KuIg <br />with Union Pacific Railroad asking for information on an invoice for a spill <br />response. Complaint number was not given in voicemail, only invoice number. <br />Called and left Anna a voicemail asking if she could please give me a call back <br />and give me the complaint number to look up as I don 't have access to the <br />invoices. <br />*Updated narrative with complaint number 11/3/2022* <br />11/03/2022 Received voicemail from Anna Kulig with UPR who provided the complaint <br />number. Looked up narratives for complaint. Looked at desk if complaint was still <br />here. Has already been sent to lead senior for review. Looked in emails for <br />information on complaint. Called and left Anna a voicemail back that the complaint <br />was for an odor reported from a railcar that UPR was waiting on EHD to <br />investigate smell and Patriot was dispatched after EHD notified we would only go <br />out after we know the materials released and that it was found to be soybean oil <br />but due to the time it took to verify there were no actual hazardous releases they <br />were billed. <br />PRESTO <br />PRESTO <br />5105.rpt Page 2 of 2
