Read through narratives and through complaint packet. PRESTO <br />Still missing manifests and some sample results based on narratives. Note on <br />front of packet that Fred Ernest was the last inspector to witness sampling and <br />program coordinator requested he update narratives. No entries by F. Ernest were <br />found. Discussed situation with lead senior. <br />03/10/2023 Reviewed narratives and noted disposal records pending. Hard to tell what was <br />already received in packet, flagged each item. Some things were out of order, <br />reordered packet. Looked up universal waste manifests listed in narratives <br />because didn't find in packet. Printed manifests out. Called Zantha Ricks, PG&E <br />and she requested an email with the address so she can check on the packet that <br />Arcadis sends with sample results. Emailed Zantha spill information. <br />PRESTO <br />03/13/2023 Received Arcadis closure report from Zantha Ricks, PG&E HAZMAT & Water PRESTO <br />Quality Supervisor on 3/10/2023. Reviewed report. <br />7/27/2021 - initial samples taken from 3 transformers. Transformer A- 650 ppm, B <br />- 7.6 ppm, C - 410 ppm. Street, gutter, storm drain, driveway, parking lot were <br />triple washed and power washed. Absorbent used and waste wash liquid were <br />collect into waste drums. <br />8/11/2021 - phase 1 sampling of asphalt, concrete, storm drain. Non-detect of <br />PCB in asphalt and concrete but storm drain wipe had a PCB of 320 ug/wipe. <br />8/28/2021 - excavation of gutter occurred due to PCB storm drain wipe. Phase 2 <br />sampling of soil from gutter excavation, based of transformer pole, concrete <br />sample of storm drain and wipe of storm drain. PCB detector is 13 out of 18 soil <br />samples, 9.9 mg/kg sample in the concrete storm drain sample and 1.6 ug/100 <br />cm2 in the wipe sample. <br />9/4/2021 - further excavation of gutter conducted due to PCBs. Samples taken in <br />gutter of excavated area and driveway. No PCBs detected. <br />3/23/2022 - verification sample taken in soil under storm drain sump. No PCBs <br />detected. <br />Disposal records received for 023134172JJK - 8/28/2021, 023134177JJK - <br />8/27/2021, 023134173JJK - 8/28/2021, 023134254JJK - 9/7/2021, 023135757JJK <br />- 3/31/2022. Not all disposal records were legible. Printed new ones from DTSC <br />website for 9/7/2021 and 3/31/2022 for file. <br />Discussed with lead senior. Removed illegible disposal records from Arcadis <br />Closure Report and replaced with clearer copies from DTSC. Removed duplicate <br />sample results. Printed out Log2 and 5105 for packet and abatement. Removed <br />all page flags and placed in lead senior in box. <br />5105 rpt <br />Page 12 of 12
