Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COGENUJ36 <br /> <br />Site Location: IN FRONT OF 1049 S WILSON WAY <br /> <br />Activity Summary <br />Activity Date <br /> Recorded by <br />02/25/2022 Printed documents including Notice to Abate, photo logs, most recent ennails, PCB MURO <br />soil sampling method. Compiled into proper complaint order. <br />02/25/2022 Received an automatic reply from Zantha Ricks yesterday, 2/24/2022 stating she <br />is out of the office through 2-25-2022. Sent another email to Zantha Ricks with a <br />cc to Lead Senior Elena Manzo and Alex Steele requesting that in addition to <br />Phase 2 lab results, EHD also needs Phase 3 lab results, Storm Drain wipe <br />sample lab results, final manifests for disposal of all generated wastes associated <br />with the spill on 7-27-2021 in order to abate this complaint. I also requested plan <br />for remediation of sidewalk, driveway, gutter, and storm drain from her, since the <br />last request for clarification via email to her she replied with cc to Alex Steele <br />stating that Alex Steele will provide, but Alex has not provided or replied to my <br />emails regarding this information. I was able to make contact with Alex Steele on <br />2/23/2022 (see narrative entry for 2/23). I requested the plan from Zantha and a <br />48-hour notice to the storm drain removal project. <br />MURO <br />03/30/2022 Arrived on site, met with Alex Steele of PG&E. Per Alex, the sample will not be <br />conducted today due to there is a water leak. Matt Stiles of California Water <br />Services (209-464-8311) who replaced the sipgot. Per Matt the vibration from the <br />equipments caused the pipe to leak and when the water pipe on the customer's <br />side was shut off, it caused the pipe to leak on the other side closes to the road. <br />Per Dave Larson, the hole will be covered up with a metal plate after the plumber <br />repair the pipe. <br />Per Alex Steele, clean up will continue on 3/31/2022 because the PG&E crew <br />received a call regarding a gas leak at another location. <br />Generated photo log <br />03/31/2022 Received notification from Elena Manzo EHD about a PG&E clean up on 1049 S <br />Wilson Way in Stockton. Sent message to Kristina Saetern EHD asking for more <br />information. Recieved phone call from Kristina S. indicating the location of the <br />spill and the contact would be Alex Steele PG&E Environmental Compliance <br />Technician. <br />SAETERN <br />ERNEST <br />Called Alex S. and he indicated he would be ready for sampling at 10:30am. <br />Arrived on site at 10:30 am, and he indicated they would not be ready for sampling <br />till later and he would call. <br />Received a call from Alex Steele at approximately 11:40am and he indicated <br />sampling would occur soon. Drove to the site, and witnessed one sample being <br />taken in native soil approximately at 6 foot of depth. Took photos of area cleaned, <br />and where samples were taken. Took another photo of the chain of custody. <br />Approximately 7 cubic feet of soil (532Ibs) of loose soil and a square concrete slab <br />approximately 3'x3' was removed from the site. Alex Steel indicated the materials <br />removed from the site will be kept at the PG&E lot of the contractors PSC's lot. <br />Created map of sampling. <br />04/01/2022 Generated photo log.
