Complaint Investigation Summary Report # 5105 COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY Activity Summary Activity Date Recorded by 09/04/2021 TIME SPENT: 8:12AM-1:00PM ALANIZ Received a voice mail from Zantha Ricks letting me know that the sampling should start around 9:00 AM. Called Jocelyn Wong and left a message. Received a call back from her. She informed me that she was on site but waiting for excavation to be completed before she could sample. Left for the site at 8:54 AM and arrived at 9:33 AM. Dave Larsen was in the process of excavating the dirt area. Witnessed sampling by Jocelyn. Took a picture of the sampling map and chain of custody. Dave generated approximately 3 yards of soil in a 20 yard roll off bin. During the excavation, Dave almost hit a buried water pipe. Left the site at 12:30 PM and got back at 1:00PM. 09/07/2021 Completed photo log and narrative from 9/4/2021 event. ALANIZ 09/07/2021 Updated Elena Manzo, lead senior, and Claudia Muro, REHS. Moved photo log to ALANIZ unit folder. 09/07/2021 Received update from John Alaniz on the PCB Phase 3 sampling. Reviewed his MURO narrative and photo log. 09/08/2021 The following took place on Fri., 9/3/2021 (my scheduled day off). MURO I received a voice message from Alex Steele (PG&E) regarding Phase 3 sampling event. I returned Alex Steele's phone call to obtain more information. Alex provided the following info: They are starting the sampling event on Saturday, Sept. 4 after 9 am They will be excavating because some samples from Phase 2 sampling event came back with PCBs less than 1 PPM They will be removing more soil; taking one 20-yard roll off bin The duration of the event is expected to be half a day 09/13/2021 Emailed Zantha Ricks (PG&E) request for analytical lab results for samples taken MURO on 8-28 and 9-4. Requested 48-hour notification for any future sampling for PCB confirmation and/or excavation. 09/27/2021 Called Alex Steele (PG&E) to ask that any future updates on this spill be sent to Lead Senior Elena Manzo. I asked if he has an update on the next phase of remediation which was replacement of the storm drain - he reported he has not heard anything. 10/13/2021 Sent an email to Alex Steele Steele, Rodney <R2Sv@pge.com> for a project status update 10/15/2021 Received an email update from: Steele, Rodney <R2Sv@pge.com> : "We are waiting on a permit from the City of Stockton to remove the storm drain sump.", sent a thank email back to Alex. 12/08/2021 Reviewed complaint no additional work was reported to the EHD, send an email to Steele, Rodney <R2Sv@pge.com> asking for a status update. 01/05/2022 Sent Alex Steele message at 209-337-8902 if PG&E is going to backfill the sidewalk area.
