Complaint Investigation Summary <br /> Report # 5105 <br />COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY <br />Activity Summary <br />Activity Date <br /> Recorded by <br />09/04/2021 TIME SPENT: 8:12AM-1:00PM <br /> ALANIZ <br />Received a voice mail from Zantha Ricks letting me know that the sampling should <br />start around 9:00 AM. Called Jocelyn Wong and left a message. Received a call <br />back from her. She informed me that she was on site but waiting for excavation to <br />be completed before she could sample. Left for the site at 8:54 AM and arrived at <br />9:33 AM. Dave Larsen was in the process of excavating the dirt area. <br />Witnessed sampling by Jocelyn. Took a picture of the sampling map and chain of <br />custody. Dave generated approximately 3 yards of soil in a 20 yard roll off bin. <br />During the excavation, Dave almost hit a buried water pipe. <br />Left the site at 12:30 PM and got back at 1:00PM. <br />09/07/2021 Completed photo log and narrative from 9/4/2021 event. ALANIZ <br />09/07/2021 Updated Elena Manzo, lead senior, and Claudia Muro, REHS. Moved photo log to ALANIZ <br />unit folder. <br />09/07/2021 Received update from John Alaniz on the PCB Phase 3 sampling. Reviewed his MURO <br />narrative and photo log. <br />09/08/2021 The following took place on Fri., 9/3/2021 (my scheduled day off). MURO <br />I received a voice message from Alex Steele (PG&E) regarding Phase 3 sampling <br />event. I returned Alex Steele's phone call to obtain more information. Alex <br />provided the following info: <br />They are starting the sampling event on Saturday, Sept. 4 after 9 am <br />They will be excavating because some samples from Phase 2 sampling event <br />came back with PCBs less than 1 PPM <br />They will be removing more soil; taking one 20-yard roll off bin <br />The duration of the event is expected to be half a day <br />09/13/2021 Emailed Zantha Ricks (PG&E) request for analytical lab results for samples taken MURO <br />on 8-28 and 9-4. Requested 48-hour notification for any future sampling for PCB <br />confirmation and/or excavation. <br />09/27/2021 Called Alex Steele (PG&E) to ask that any future updates on this spill be sent to <br />Lead Senior Elena Manzo. I asked if he has an update on the next phase of <br />remediation which was replacement of the storm drain - he reported he has not <br />heard anything. <br />10/13/2021 Sent an email to Alex Steele Steele, Rodney <R2Sv@pge.com> for a project <br />status update <br />10/15/2021 Received an email update from: Steele, Rodney <R2Sv@pge.com> : "We are <br />waiting on a permit from the City of Stockton to remove the storm drain sump.", <br />sent a thank email back to Alex. <br />12/08/2021 Reviewed complaint no additional work was reported to the EHD, send an email to <br />Steele, Rodney <R2Sv@pge.com> asking for a status update. <br />01/05/2022 Sent Alex Steele message at 209-337-8902 if PG&E is going to backfill the <br />sidewalk area.
