Complaint Investigation Summary Report #: 5105 COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY Activity Summary Activity Date Recorded by 01/19/2022 Sent an email to Zantha Ricks (PG&E) requesting an update and asked if there MURO will be confirmation sampling for the storm drain area during the replacement. Asked if they have scheduled a date scheduled for the sidewalk and driveway construction. 01/26/2022 Organizing files (emails, narrative docs, reports) into a single folder. MURO 01/27/2022 Sent an email to Allexis Illich (US EPA) Illich.Alexis@epa.gov since she is the person who was working with PG&E on the PCB spill. I let her know the site is still secured, but has not had any further development and asked if she had any information on the cleanup project. I expressed concerns on the information received by Alex Steele stated they will not conduct anymore sampling during the replacement of the sidewalk and driveway, and probably not sampling the soil at the time the storm drain is replaced. I contacted Zantha Ricks, but she has not responded. MURO 01/31/2022 Received reply from Zantha Ricks. She stated she asked Alex Steele to provide MURO update. 02/02/2022 Completed three photo logs for complaint. MURO 02/10/2022 On my way to an unrelated inspection, observed right southbound lane on Wilson MURO Way was closed due to construction on PCB spill site. Notified and discussed with Elena Manzo. According to Alex Steele, the last time he updated status for cleanup project was that the sidewalk and drive way will be replaced. No sampling to occur during this procedure due to analytical lab result finding the two areas to be non-detect. I will call Alex to follow up on the project. 02/23/2022 Called Alex Steele inquiring about the cleanup project. He said for now they (Arrow MURO Construction) are only working replacing the side walk and driveway. He anticipates concrete will be done on Friday, 2-25-2022. Then, PG&E will address the storm drain. No sampling is taking place during the sidewalk and driveway construction. He said he will let me know when they start to work on the storm drain. I let him know I will be handing this complaint over to Lead Senior Elena Manzo, so he will need to contact her. 02/24/2022 Sent an email to Zantha Ricks (PG&E) with a cc to Lead Senior Elena Manzo and MURO Alex Steele (PG&E) letting her know that I am no longer overseeing the cleanup/remediation for this complaint. Requested Phase 2 sample results. 02/24/2022 Entered incomplete narrative from the field notes I gathered. Organized complaint MURO documents before submitting to Lead Senor Elena Manzo. 02/25/2022 After thorough review of the narrative yesterday, I left out some items still needed. MURO Emailed Zantha Ricks (PG&E) requesting the following: Disposal records and final manifests for cleanup on dates 7-27-2021, 8-28-2021, 9-4-2021, all future disposal records generated from the remediation project, and any additional cleanup not listed here. Phase 2 and Phase 3 confirmation sample results, and the storm drain wipe sample results from Phase 1, all future lab analysis reports, and any additional sampling not listed here.
