Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COBOW2LCW Site Location: 23975 E MILTON RD LINDEN CA 95236 <br />Activity Summary <br />Activity Date Recorded by <br />08/02/2021 Analyzed complaint and decided to visit site with RENS John Alaniz and Elena NSO <br />Manzo, lead Senior REHS. On site <br />met with office Manager, Kylie Spingola and inspected property and took photos. <br />We observed a rusted car placed onto soil. The car was located at the north end <br />of the pesticide storage warehouse near a CAL-WASTE 40-yard roll-off bin. The <br />car was partially dismantled and covered with soil. Dark oily staining was also <br />observed on the soil near the silo and parked trucks. One pallet of Peters Excel 15 <br />by 5 by 15 observed in Greenhouse 3 D had material that had leaked down the <br />sides of the cardboard. Observed the following hazardous materials over the <br />threshold limit for the Hazardous Material Business Plan (HMBP) program (exact <br />quantity unknown): <br />-Propane (two large tanks and several smaller cylinders for the forklifts) <br />-Nutricote Total 18-6-8 Type 140 (5.5 pallets). <br />08/02/2021 Site visit, assisted with facility inspection. Met with Kylie Spingle (office manager), MANZO <br />who granted permission to conduct a facility walk through and escorted myself, J. <br />Alaniz and P. Nso on a facility inspection. Kylie indicated that she will be going on <br />maternity leave later this week. Kylie indicated that cars were buried onsite some <br />time last season. She indicated that cars belonged to the former facility manager <br />(Randy) . When the manager left, Miguel Delamadrid (regional manager <br />619-664-6921) directed facility to bury them onsite. Recently Miguel directed them <br />to un bury the vehicles. Kylie indicated that Chad Tompkins, no longer with the <br />company, buried these cars. Observed a car which was unburied with some oily <br />dripping from the inside car parts. Several chemicals in quantities over 500 lbs of <br />solids were observed onsite, facility needs to be added to the business plan <br />program. See J. Alaniz narrative for more details. <br />08/02/2 021 Reviewed complaint information concerning three vehicles which were buried on MANZO <br />the property. Facility is on inventory for employee housing only. <br />08/03/2021 Completed the notice to abate. Emailed it to Kylie Spingola at ALANIZ <br /><kylie.spingola@bonnieplants.com> and the regional manager, Miguel Madrid, at <br /><miguel.madrid@bonnieplants.com>. <br />Called Miguel at 619-664-6921 and confirmed he received it. Answered several <br />questions he had. Called Kylie at 661-332-5075, and confirmed she received it. <br />Kylie said the complaint would be taken over by the safety director, Steve <br />Harmon, (334) 421-3114. Emailed the notice to the complainant and answered <br />several questions they had regarding the follow up. <br />08/16/2021 Received a call from Donnie Lance, 256-221-0815. He is a consultant for ALANIZ <br />Enersolv Corporation hired by Bonnie Plants. Donnie said he was preparing a <br />response to my notice to abate and would send it to me within the next few days. <br />09/09/2021 Received a call from Matt Livingston with Atlas Consultants, 9250786-1122. Matt ALANIZ <br />informed me that the property was in the process of being EMF surveyed to <br />determine if there are any buried cars. <br />10/12/2021 Received a proposal for the cleanup from Matt Livingston ALANIZ <br /><matt.livingston@atcgs.com> on 10/12/2021. The proposal referenced a site map <br />that was not included in the email. Called Matt at 925-786-1122 and asked him to <br />send me the site map. Matt said he would send it to me on 10/13/2021. He also <br />said he was planning on coming into the EHD next week to take out a permit for <br />the soil borings. <br />10/13/2021 Discussed cleanup with Elena Manzo, lead senior.
