Complaint Investigation Summary Report #: 5105 COMPLAINT ID: COBOW2LCW Site Location: 23975 E MILTON RD LINDEN CA 95236 Activity Summary Activity Date Recorded by 08/02/2021 Analyzed complaint and decided to visit site with RENS John Alaniz and Elena NSO Manzo, lead Senior REHS. On site met with office Manager, Kylie Spingola and inspected property and took photos. We observed a rusted car placed onto soil. The car was located at the north end of the pesticide storage warehouse near a CAL-WASTE 40-yard roll-off bin. The car was partially dismantled and covered with soil. Dark oily staining was also observed on the soil near the silo and parked trucks. One pallet of Peters Excel 15 by 5 by 15 observed in Greenhouse 3 D had material that had leaked down the sides of the cardboard. Observed the following hazardous materials over the threshold limit for the Hazardous Material Business Plan (HMBP) program (exact quantity unknown): -Propane (two large tanks and several smaller cylinders for the forklifts) -Nutricote Total 18-6-8 Type 140 (5.5 pallets). 08/02/2021 Site visit, assisted with facility inspection. Met with Kylie Spingle (office manager), MANZO who granted permission to conduct a facility walk through and escorted myself, J. Alaniz and P. Nso on a facility inspection. Kylie indicated that she will be going on maternity leave later this week. Kylie indicated that cars were buried onsite some time last season. She indicated that cars belonged to the former facility manager (Randy) . When the manager left, Miguel Delamadrid (regional manager 619-664-6921) directed facility to bury them onsite. Recently Miguel directed them to un bury the vehicles. Kylie indicated that Chad Tompkins, no longer with the company, buried these cars. Observed a car which was unburied with some oily dripping from the inside car parts. Several chemicals in quantities over 500 lbs of solids were observed onsite, facility needs to be added to the business plan program. See J. Alaniz narrative for more details. 08/02/2 021 Reviewed complaint information concerning three vehicles which were buried on MANZO the property. Facility is on inventory for employee housing only. 08/03/2021 Completed the notice to abate. Emailed it to Kylie Spingola at ALANIZ <kylie.spingola@bonnieplants.com> and the regional manager, Miguel Madrid, at <miguel.madrid@bonnieplants.com>. Called Miguel at 619-664-6921 and confirmed he received it. Answered several questions he had. Called Kylie at 661-332-5075, and confirmed she received it. Kylie said the complaint would be taken over by the safety director, Steve Harmon, (334) 421-3114. Emailed the notice to the complainant and answered several questions they had regarding the follow up. 08/16/2021 Received a call from Donnie Lance, 256-221-0815. He is a consultant for ALANIZ Enersolv Corporation hired by Bonnie Plants. Donnie said he was preparing a response to my notice to abate and would send it to me within the next few days. 09/09/2021 Received a call from Matt Livingston with Atlas Consultants, 9250786-1122. Matt ALANIZ informed me that the property was in the process of being EMF surveyed to determine if there are any buried cars. 10/12/2021 Received a proposal for the cleanup from Matt Livingston ALANIZ <matt.livingston@atcgs.com> on 10/12/2021. The proposal referenced a site map that was not included in the email. Called Matt at 925-786-1122 and asked him to send me the site map. Matt said he would send it to me on 10/13/2021. He also said he was planning on coming into the EHD next week to take out a permit for the soil borings. 10/13/2021 Discussed cleanup with Elena Manzo, lead senior. ALANIZ 510.rpt Page 2 of 4
