COUNLY <br />(.111r-or <br />4 •V" Greatness grows hers-. <br />Environmental Health Department <br />Time In: 12:24 pm <br />Time Out: 1:24 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility DENIS COUNTRY KITCHEN Date: 02/28/2023 <br />Address 1327W LOCKEFORD ST, LODI 95240 <br />Owner/Operator: AHMADI, ABDUL WAHID Telephone (209) 368-6350 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00055575 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT STATES THE RESTAURANT IS BEAT UP AND THE FRIDGE HAS HEALTH VIOLATIONS. <br />COMPLAINANT STATES MEAT IS STORED WITH CHEESE, MEAT IS STORED WITH VEGETABLES, AND MEAT IS <br />DRIPPING ONTO VEGETABLES. COMPLAINANT STATES AN EMPLOYEE GETS UPSET AND BREAKS PLATES AND <br />SOME OF THE BROKEN PLATE PARTICLES GET INTO THE FOOD. COMPLAINANT STATES THE GARBAGE <br />DISPOSAL IS NEVER CLEANED AND BACKS UP. DISHWASHER NEEDS FIXED AND BACKS UP. COMPLAINANT <br />STATES EMPLOYEES PUSH WATERY FOOD INTO THE STREET, COMPLAINANT SUGGESTS LOOKING OUTSIDE IN <br />THE BACK AREA - THERE ARE MOPS THAT ARE MESSED UP AND HAVE PARTICLES IN THEM. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />#6 Handwashing Facilities Supplied and Accessible <br />OBSERVATIONS: The paper towel dispenser was empty at the restroom hand sink, paper towels were on top of the <br />dispenser. Provide paper towels in dispenser today. <br />The paper towel dispenser at the kitchen hand sink was not working, paper towel was on prep table. Provide working paper <br />towel dispenser today. <br />CALCODE DESCRIPTION: Handwashing soap and towels or drying device shall be provided in dispensers dispensers shall be <br />maintained in good repair. (113953.2) Adequate facilities shall be provided for hand washing, food preparation and the washing of <br />utensils and equipment. (113953, 113953.1, 114067(1)) <br />#7 Hot and Cold Holding Temperatures <br />OBSERVATIONS: The chef drawers were 44 F, the 2 D Blue Air was 44 F, and the 2 D Coca Cola display was 46 F. <br />Provide 41 F or below today. <br />CALCODE DESCRIPTION: Potentially hazardous foods shall be held at or below 41/ 45°F orator above 135°F.
