Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COTR7GWOA Site Location: 2829 W WASHINGTON ST, STOCKTON CA 95203 <br />Activity Summary <br />Activity Date Recorded by <br />02/08/2023 Received the following email from Luke Allen on 2/8/23: GARCIA-MEJIA <br />"Hello Gabriela, <br />On January 26th and 27th Ponder environmental pumped 16 totes of the <br />acid/water mixture back into our product tank (TK100). Attached is the work order <br />Ponder used while completing the tote transfer project. All acid/water mixture from <br />the incident has been returned to product. If where is anything else you might <br />need, please don't hesitate to ask. Thanks Gabriela." <br />In his email he included 2 attachments of the work orders from Ponder for the <br />transfer of the liquid solution. Printed forms needed to abate the complaint. While <br />reading the complaint narratives I saw that a 65-gallon tote of used PPE was <br />generated during the clean up. I asked Luke if that drum of waste has been <br />disposed of already and if so I asked that he provide me a copy of the disposal <br />record. <br />I received the following reply from Luke "lye been in contact with Ponder <br />Environmental, they are picking that tote up. Ponder needed to ensure we had the <br />correct profile for disposal. As soon as I have the disposal paper work I'll send that <br />to you. Thanks Gabriela." <br />03/10/2023 Received a copy of the manifest for the drum that was disposed of on 3/9/23. GARCIA-MEJIA <br />Manifest number 023407816JJK. Printed out manifest and added it to the file. <br />Emailed Luke Allen <Iallen@satcoinc.net> asking him to send me the final signed <br />copy once he receives it. Complaint can be abated once the final signed copy of <br />the manifest is obtained. <br />04/17/2023 Received copy of the final signed manifest via email from Luke Allen GARCIA-MEJIA <br /><Iallen@satcoinc.net> on 4/17/2023. Printed out all necessary forms. No <br />emergency response record was in my files. Emailed the inspector that was on <br />site regarding the ER record. Received ER record and abated the complaint. <br />04/17/2023 Entered information for Persons at Scene and generated ER01 and site map. PRESTO <br />Printed and gave to assigned inspector. <br />5105 rpt <br />Page 3 of 3
