Submit spore testing logs to the Environmental Health <br />Department (EHD). <br /> <br />#5. HSC 119314 - Decontamination/sanitation area separated and supplied appropriately <br />OBSERVATION: The sterilization room was not supplied with paper towels. <br />CORRECTIVE ACTION: The decontamination and sterilization areas shall be equipped with a sink, hot and cold running water, <br />containerized liquid soap, and single-use paper towels dispensed from a wall-mounted, touchless dispenser that is readily <br />accessible to the practitioner. Supply the sink in the sterilization room with single-use paper towels and submit photo(s) to the <br />EHD as evidence of the correction. <br /> <br />#6. HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATION: <br />1. Sterilization certificates were not available for Brianna’s needles (Cheyenne Lot#2022-22-017 and 1205RL Lot#10-04- <br />2029) <br />2. Sterilization certificates were not available for piercing needles (Precision Needles Lot #240825 and #210515) <br />3. Invoices were not available for piercing jewelry. <br />CORRECTIVE ACTION: <br />1. Submit sterilization certificates to EHD for the lot numbers listed above. <br />2. Submit sterilization certificates to EHD for the lot numbers listed above. <br />3. Submit invoices to the EHD for jewelry purchase. <br /> <br />#7. HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATION: <br />1. The sharps container in the piercing station is full. <br />2. Disposal records for the last 3 years were not available at the time of the inspection. <br />CORRECTIVE ACTION: <br />1. Replace the sharps container in the piercing station and property dispose of the full container. Submit photo(s) as <br />evidence of the correction and provide disposal record for the full container. <br />2. Provide sharps disposal records for the last 3 years. <br /> <br />#9. HSC 119309 - No eating, drinking or smoking - clean clothes <br />OBSERVATION: A water bottle was observed in Kyle’s procedure room. <br />CORRECTIVE ACTION: No food, drink, tobacco product, or personal effects are permitted in the procedure are. This violation <br />was corrected at the time of the inspection. <br /> <br />#11. HSC 119314 - Handwashing facilities properly supplied and accessible, warm water <br />OBSERVATION: The restroom does not have paper towels in the wall-mounted dispenser. A countertop paper towel dispenser <br />was in use at the time of the inspection. <br />
