Discontinue the use of <br />the counter-top paper towel dispenser and restock the wall-mounted paper towel dispenser. Submit photos to the EHD. <br /> <br />#18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: <br />1. Brianna – some client records were missing the medical questionnaire. <br />2. Koedy – some client records are missing the client’s signature and/or artist name <br />3. Piercing client records – all records are missing lot numbers. Some records are missing minor IDs and/or guardian IDs <br />4. Piercing consent forms – missing statement regarding the permanent nature of the procedure <br />5. Piercing medical questionnaire – missing questions regarding (1) allergies to latex, (2) allergies to antibiotics, (3) current <br />medications, (4) requirements for antibiotics prior to surgery or dental procedures, (5) other risk factors of bloodborne <br />pathogens <br />CORRECTIVE ACTION: <br />1. Prior to the performance of body art, the client shall receive, complete, and sign a questionnaire. Implement moving <br />forward. <br />2. Prior to the performance of body art, the client shall read, complete, and sign an informed consent form. Implement <br />moving forward. <br />3. Ensure to document lot numbers of all pre-sterilized instruments used for each procedure, and IDs for minors and <br />guardians for all piercings. Implement moving forward. <br />4. Update the piercing consent forms and submit the revised copy to the EHD. <br />5. Update the piercing medical questionnaire and submit the revised copy to the EHD. <br /> <br /> #32. HSC 119314 - Floors and walls clean and in good repair, adequate light <br />OBSERVATION: <br />1. The floor in Koedy’s procedure area and outside Kyle’s procedure room is damaged. <br />2. The wall in the piercing station has peeling paint. <br />3. The doorway into Natalie’s room has holes. <br />CORRECTIVE ACTION: <br />1. A body art facility shall have floors and walls that are smooth, nonabsorbent, free of open holes and washable. Repair <br />the damaged floors. Submit photos to the EHD. <br />2. Repair with gloss finish paint. Submit photos to the EHD. <br />3. Repair holes in the walls. Submit photos to the EHD. <br /> <br />#33. HSC 119314 - Workstation, surfaces, including chairs, armrests, etc. in good repair <br />OBSERVATION: The procedure chair in the piercing station is damaged. <br />CORRECTIVE ACTION: Repair (patching not acceptable) or replace the procedure chair and submit photos to the EHD. <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: Infection Prevention Control Plan (IPCP) training records were missing for Carlos Hernandez and James <br />(Mikey) Langley. <br />CORRECTIVE ACTION: Provide IPCP training to Carlos and James and submit the training record to the EHD. <br /> <br />#37. HSC 119312 - Permits obtained and available <br />OBSERVATION: James (Mikey) Langley is currently registered as a practitioner with Stanislaus County. Per the piercing <br />client records on-site, James has performed piercings in San Joaquin County for more than 5 consecutive days and more <br />than 15 days total this calendar year. <br />CORRECTIVE ACTION: A valid and current registration issued by a local enforcement agency shall be valid in any other <br />jurisdiction for no more than 5 consecutive days, or 15 days total, in any one calendar year. James’ Stanislaus County <br />registration is no longer valid outside of their jurisdiction. James must register as a practitioner in San Joaquin County <br />immediately.
