COMPLAINANT <br />STATES THAT ON 04/18/2023 HE ORDERED A SHRIMP BOWL WITH OUT SOUR CREAM FROM THE <br />ESTABLISHMENT. COMPLAINANT STATES HE ATE AROUND 4 PM AND BEGAN HAVING STOMACH CRAMPS <br />AROUND 7 PM ROUGHLY 2-3 HOURS LATER COMPLAINANT SAYS HE STARTED HAVING DIARRHEA. <br />COMPLAINANT STATES THE NEXT MORNING HE BEGAN VOMITING AS WELL AS CONTINUED DIARRHEA. <br />COMPLAINANT STILL HAS DIARRHEAAND DID NOT SEE A DOCTOR." <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />#61 Lack of Approval by Enforcement Agency <br />OBSERVATIONS: Food truck is currently inactive (PR0545112). <br />A permit is required to operate. <br />Obtain a service request change of ownership consultation inspection immediately. <br />Food truck may not operate without a permit. <br />CALCODE DESCRIPTION: The mobile food facility has not been approved by the enforcement agency.
