COMPLAINANT STATES THAT AROUND 1:00 AM ON 04/22/2023 HE FOUND HIS <br />WIFE ON THE FLOOR PARALYZED. COMPLAINANT STATES HE TOOK HIS WIFE TO THE HOSPITAL WHERE SHE <br />WAS DIAGNOSED WITH A BACTERIAL FOOD POISONING CAUSED BY SHELL FISH. COMPLAINANT ALSO STATES <br />HIS WIFE IS STILL SICK, CAN'T HOLD ANYTHING DOWN, AND IS CURRENTLY ON ANTIBIOTICS. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7: <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodbome illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM --LOCATION -- TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />COMMENTS AND OBSERVATIONS <br />Complaint inspection. No major violations. No re-inspection. <br />Operator unaware of complaint. No similar complaints received. No employees reported sickness. Food safety training <br />lacking. Food manager certificate and food handler cards on site expired. Kitchen hand washing station complete with warm <br />and cold water, soap and paper towels. Sanitizer buckets set up at each station with proper concentration of Sink and <br />Surface Cleaner Sanitizer. Both dishwashers observed with chlorine sanitizer concentration at 50 PPM. Employee restroom <br />lacked soap and the toilet paper dispenser was in disrepair. <br />Oysters are received under refrigeration and stored in the walk-in cooler and prepared when ordered by a customer. <br />Temperature of the walk-in cooler was observed at 39 F. Oysters are served raw and cooked. Raw oysters are prepared by <br />being cleaned and shucked at the prep sink and served to customer on a tray of shaved ice. Cooked oysters are prepared by <br />being cleaned andshucked. Oysters are then baked and served to customer on salt. Over three months of oyster tags <br />available on site. <br />Discussed inspection report with Jarrod Tillery (Manager).
