The incorrect and missing information included the following from the inspection reports: <br />- The 3/10/2023 DO report did not include owner/operator name <br />- The 9/26/2022 DO report did not include the designated operator name. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />This was corrected on site. <br />This is a minor violation. <br /> 306 HSC 25291(e) Failure to remove, analyze, and properly dispose of water in secondary containment (pre-Jul 2003). <br />OBSERVATION: Liquid was observed in the 91 and 87 fill sumps. <br />REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br />must be removed and disposed of properly. <br />CORRECTIVE ACTION: The service technician removed approximately 40.5 gallons of liquid from the 91 and 87 fill <br />sumps.
