The following is an itemized list of underground storage tank violations that have not <br /> been addressed for ARCO AMPM as of October 30, 2023. <br /> Open violations from June 27, 2023 inspection <br /> Violation#118-Failure to submit the"Overfill Prevention Equipment Inspection Report Form"within 30 <br /> days. <br /> OBSERVATION: No record of overfill prevention testing was observed on site or received by EHD. <br /> REGULATION GUIDANCE: Overfill prevention equipment testing is required once every 36 months. A copy of the <br /> test report must be submitted within 30 days of the test. <br /> CORRECTIVE ACTION: Immediately schedule this test and provide 48 hours notification to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#203-Failure to maintain monitoring records for 36 months and maintenance records for 60 <br /> months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -Overfill prevention equipment test results were not found on site during the inspection. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#205-Failure to submit and have current UST Monitoring Plan readily accessible at the facility. <br /> OBSERVATION:An approved copy of the monitoring plans were not available for inspection. <br /> REGULATION GUIDANCE:A copy of these plans shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> Page 1 of 2 <br />
