Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> COMMENTS AND OBSERVATIONS <br /> Manager is on site and is aware of the complaint. Per Manager,the facility had an issue with cockroaches recently and is <br /> actively working on it. Per Manager,the facility contracted with ProActive Pest Control around 45 days ago. ProActive services <br /> the facility weekly. <br /> At the time of the inspection, ProActive arrived on site to conduct service. Non-food prep areas were sprayed, bait was <br /> applied and glue boards were changed out. Live German cockroaches were observed on the glue boards, but at no other <br /> location within the facility.
