Complaint Investigation Summary Report #: 5105 COMPLAINT ID: C031SZOIY Site Location: 720 S B ST Activity Summary Activity Date Recorded by 5: 42 p.m.: Called Cory. He said that Jose would call us when he was done cleaning the area. It had taken a couple of hours to get the train moved. 5:54-6:06 p.m.: Received a call from Jose. He said the cleanup had been completed. Contaminated rocks were taken off of the track. Jose sent me photos of the final cleanup. The clean-up was good. Discussed with Melissa Nissinn, program coordinator, and it was determined I would not need to go to verify final cleanup since I was going to the site the next day. 02/22/2021 8:12 a.m.: Received photos of the cleaned up area from Cory Clapp. Called him ALANIZ at 720-417-3873, and informed him I was leaving to the site. The area was properly cleaned up. Four 55 gallon drums of diesel contaminated water and three 55 gallon drums of diesel contaminated solids were generated. Left the site and arrived back to the EHD at 9:06 a.m. Time stamped the photos taken on 2/22/2021. Created photo logs for photos taken on 2/21 and 2/22. Finished filling out the complaint information and completed a PROP 65. Reviewed the safety data sheet for the Micro Blaze. Completed narratives for 2/21 and 2/22. 02/22/2021 Reviewed complaint with Elena Manzo. ALANIZ 03/08/2021 Received uniform manfiest for disposal of waste on 3/5/2021 from Cory Clapp ALANIZ <cory.clapp©velocityrail.com>. Sent him an email asking for the final copy when he received it. 03/17/2021 Searched for final manifest 006457146GBF in RCRA Info and DTSC. Could not ALANIZ find final TSDF signed copy. 05/09/2023 Printed the final disposal record from DTSC. Gathered the complaint documents ALANIZ and abated the complaint.
