Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COEMHN7Z6 Site Location: 3837 IMPERIAL WAY, STOCKTON <br />Activity Summary <br />Activity Date Recorded by <br />03/30/2021 Sent an email discussing the incident with lead seniors. ALANIZ <br />Sent an email of spill reporting requirements to Danette Rodriguez at <br /><Danette.Rodriguez@mcam.com>. <br />Discussed facility with Garrett Backus, lead senior. <br />03/31/2021 Received an email from Danette Rodriguez on 3/30/2021 saying <br />"I spoke with RB Environmental and they told me that what is in the drum is <br />everything they picked up. They did not us a vacuum style machine just a scrubber <br />machine and they disposed of everything in the container. Is this all the <br />information you need other than the manifest?" <br />Called her back. She said was in a meeting. It was agreed she would call me at <br />1:00 p.m. <br />Discussed with Garrett Backus, lead senior. <br />Called Jerry Lynch, general manager with Alco Metals, 510-908-6570. Discussed <br />the event with him. He said that he had not been informed of the incident but that <br />he would look into it. <br />Called Danette. She said that there was no power washer used in the final <br />cleanup. The facility placed a chemical down with the absorbent and cleaned the <br />area used a metal scrubber. All of the waste generated was placed into the 55 <br />gallon drum observed on site. I asked her to send me an email providing the <br />explanation. <br />Sent an email to Jerry Lynch, <jerrylynch@alcometals.com>, asking the following <br />questions: <br />Were you able to find out any more information on this incident? <br />What is Alco's process to ensure that this does not happen? <br />Are the employees trained on inspecting containers before they are dropped <br />off? If so, are there any training documents you could provide showing this? <br />Discussed complaint with Elena Manzo, lead senior. <br />04/01/2021 Received an email and call from Michael Bercovich, <br /><michaelbercovich@alcometals.com>, discussing Alco's response to the incident. <br />His phone number is 510-562-1107. <br />04/08/2021 Reviewed Alco's response. It was acceptable. The response also included <br />training records. <br />Discussed billing with Elena Manzo, lead senior. <br />04/13/2021 Received an email from Danette Rodriguez, < Danette.Rodriguez@mcam.com>, <br />on 4/12/2021 asking about the PROP 65. She also attached a receipt for the <br />disposal of the waste. Called her at 209-561-5700, and asked for the disposal <br />record. She said she would get back to me. <br />ALANIZ <br />ALANIZ <br />ALANIZ <br />ALANIZ <br />04/15/2021 Received a manifest for the waste cleanup from Dannette Rodriguez. Sent a reply ALANIZ <br />back asking for a destination facility signed copy. <br />05/09/2023 The waste from the incident was disposed of as hazardous waste. Printed a ALANIZ <br />TSDF signed copy of the manifest from DTSC's website. The site map was <br />created using photos from the site visit. Made corrections to complaint. <br />05/10/2023 Gathered inspection documents and abated the complaint. ALANIZ <br />5105 rpt Page 2 of 2
