HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATIONS: <br />Sterilization certificates were not available for Petera Fareas’ Precision needles with the lot #240610. <br /> <br />CORRECTIVE ACTIONS: <br />A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and c lient, <br />date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. Provide EHD with a sterilization certificate for the instrument mentioned above. <br /> <br />7. HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATIONS: <br />Per facility owner, Wesley Fareas, sharps waste was disposed of at a local sharps waste drop off location. A sharps waste <br />hauler or mail back system has not been set up. <br /> <br />CORRECTIVE ACTION: <br />Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. Documentation of proper <br />disposal shall be maintained for 3 years. Provide EHD with a contract from a sharps waste hauler or an approved sharps waste <br />mail-back system. <br /> <br />9. HSC 119309 - No eating, drinking or smoking - clean clothes <br />OBSERVATIONS: <br />A water bottle was observed next to the printer. <br /> <br />CORRECTIVE ACTIONS: <br />No food, drink, tobacco product, or personal effects are permitted in the procedure area. Remove the water bottle from the <br />procedure area and ensure there is no food or drink in the area. <br /> <br />13. HSC 119307 - Bloodborne Pathogen training <br />OBSERVATIONS: <br />Vernon Ring did not have a current Bloodborne Pathogen (BBP) Training Certificate. Per Mr. Ring, he took Cathy Montie’s BBP <br />Training course but is unable to get the certificate until he submits a valid ID, which is expired. He is in the process of obtaining a <br />current ID. <br /> <br />CORRECTIVE ACTIONS: <br />Practitioner shall provide evidence of a completed, EHD approved, OSHA Bloodborne Pathogen Training consistent with section <br />119307. Mr. Ring is to take a different course if he is unable to obtain a certificate from Cathy Montie’s course. Submit a current <br />BBP Training Certificate to the EHD for Vernon Ring. <br /> <br />18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATIONS: <br />The client records were not complete and photo IDs were not attached to the records. <br />
