Ensure all client records are complete and photo ID of the client is attached. <br /> <br />33. HSC 119314 - Workstation, surfaces, including chairs, armrests, etc. in good repair <br />OBSERVATIONS: <br />1. Damaged chairs and stools were observed at Jose Hernandez’s workstation, Vernon Ring’s workstation, Petra Farea’s <br />workstation, at the drawing station and at the reception desk. <br />2. Absorbent items observed at Jose Hernandez’s old workstation, next to the restroom (hanging shirts) and on a cabinet <br />between the handwashing sinks. <br /> <br />CORRECTIVE ACTIONS: <br />1. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good <br />repair and capable of being disinfected. Repair or replace all chairs and stools that are damaged. Provide EHD with <br />photographic evidence. <br />2. HSC 119313 - Operation and employee training records present <br />OBSERVATIONS: <br />The training record provided was from April 2022. <br /> <br />CORRECTIVE ACTIONS: <br />Operation and employee training records shall be maintained for 3 years and made available for inspection. A written Infecti on <br />Prevention and Control Plan (IPCP) shall be maintained, followed and updated. Practitioners are to be trained on the IPCP <br />annually or when changes are made to the IPCP. Train all practitioners on the IPCP and provide EHD with a current training <br />record. <br /> <br />37. HSC 119312 - Permits obtained and available <br />OBSERVATIONS: <br />Facility owner is not informing the LEA of new practitioners joining the facility or of practitioners leaving the facility. <br /> <br />CORRECTIVE ACTIONS: <br />A body art facility shall notify the LEA in writing within 30 days of resignation and/or termination of a practitioner. A new hire or <br />apprentice must be registered with the EHD prior to performing body art at the facility. <br /> <br /> <br /> <br /> <br /> <br /> <br />Correct all violations and provide evidence of corrections to the EHD <br />IMMEDIATELY! <br /> <br /> <br />
