Inspection report emailed to facility owner, Wesley Fareas, at deathraytattoo@gmail.com. <br />2. Facility plans to utilize Sharps Compliance (mail-back system) for sharps waste disposal. <br />3. The Smartwaiver program is used for client records. Per Petra Fareas, the facility began using the program in 2024. <br />Currently, the facility is unable to upload/ attach client photo IDs to the Smartwaiver program. Facility will have to <br />discontinue using the program if this issue is not resolved. Hard copies of client records are kept at the site from 2024 <br />and prior years. <br />4. The facility is transitioning from using Madacide to using Preempt disinfectant. Ensure Madacide is replaced with <br />Preempt in the Infection Prevention and Control Plan. <br />5. Jose Hernandez is no longer practicing at the facility, per Mr. Hernandez. Facility owner did not communicate this with <br />the LEA. The facility owner is responsible for informing the LEA of practitioners leaving the facility (see item # 37). <br />6. All of the routine inspection violations were to be corrected by February 7, 2025, but the LEA has not received any <br />corrections. Wesley Fareas was contacted on March 19, 2025 for the corrections. He stated the corrections would be <br />submitted by March 21, 2025, but the LEA has not received any submittals. If corrections are not submitted <br />immediately, then a reinspection may be conducted at a rate of $172/hr. <br />7. Disinfectant bottles were observed with proper labels.
